摘要
目的探讨经皮肾技术Ⅰ期治疗肾盂旁囊肿合并肾结石的疗效和安全性。方法2008年1月-2018年12月间共对29例被诊断为肾盂旁囊肿合并肾结石的患者采用经皮肾技术Ⅰ期治疗,记录手术成功率、术中和术后以及随访2年的并发症等。结果所有患者均成功Ⅰ期同时完成MPNL和肾盂旁囊肿内切开引流术,无死亡病例,术中和术后以及随访期间未见严重并发症发生。25例(86.2%)PCNL一次性取净结石,联合应用Ⅱ期手术或SWL等辅助措施后总净石率达100%。术后3个月18例患者囊肿消失,11例患者囊肿明显缩小,其中7例患者囊肿缩小至原体积的20%、4例患者囊肿缩小至原体积的30%。结论经皮肾技术Ⅰ期治疗肾盂旁囊肿合并肾结石具有疗效确切、创伤小、恢复快和并发症少的优点,可作为安全有效的方法临床推广。
Objective To explore the efficacy and safety of percutaneous technology for simultaneous treatment of peripelvic renal cyst complicated with renal calculi.Methods From January 2008 to December 2018,a total of 29 patients were diagnosed as peripelvic renal cyst complicated with renal calculi.All the patients received the ultrasound-guided MPNL procedure and internal drainage technology due to the peripelvic renal cyst complicated with renal calculi.The success rate of the operations,intraoperative,postoperative and 2-years follow-up complications were recorded in detail.Results The MPNL procedure and internal drainage technology was simultaneously completed in all patients for the treatment of peripelvic renal cyst complicated with renal calculi without any death and serious complictions in intraoperative,postoperative and 2-years follow-up period.The residual stones were not found in 25 patients after the first MPNL,and the total stone free rate was 100%after secondary operation or SWL.After three months postoperatively,the cysts disappeared in 18 patients,and shrank significantly in 11 patients,including 7 patients to 20%of the original volume and 4 patients to 30%of the original volume.Conclusion Percutaneous technology was a feasible,safe and effective method for simultaneous treatment of peripelvic renal cyst complicated with renal calculi.
