摘要
目的探讨妊娠期糖尿病患者中的早孕期血清同型半胱氨酸及维生素含量的临床意义,为早孕期预防妊娠期糖尿病提供参考,有助于降低妊娠期糖代谢异常对母婴的影响。方法本次试验的研究对象为2019年6月至次年6月一年间在我院进行治疗的50名妊娠期糖尿病病患并将之归为观察组,与此同时另选取50名该时段内的正常妊娠期妇女50名并将之归为对照组,针分别对两组(tHcy)、VB6、VB12、叶酸含量进行回顾性分析。结果除tHcy水平外,其余三项水平对比,观察组均明显低于对照组,基于观察组糖化血红蛋白分析,并将该组划分成血糖控制不理想组(G1)及血糖控制良好组(G2)(未考虑是否胰岛素或药物控制血糖),G1组叶酸水平明显低于G2组,G1组患者的VB6、VB12水平与G2组患者无统计学差异,G1组患者的tHcy水平则显著高于G2组患者。结论血清tHcy、叶酸等能够影响妊娠糖尿病的发展,早孕期维生素补充,尤其是叶酸补充,能改善妊娠期糖尿病患者临床预后,从而降低糖尿病引起的胎儿发育异常和子代近期及远期并发症。
Objective:This research is to explore the clinical significance of serum homocysteine and vitamin content in early pregnancy in patients with gestational diabetes,and to provide references for the prevention of gestational diabetes in early pregnancy,Thereby reducing the influence of abnormal glucose metabolism during pregnancy on mother and infant.Methods:This research selects the pregnant women admitted from June 2019 to June 2020 in our hospital。Fifty patients with gestational diabetes mellitus were classified as the observation group.At the same time,another 50 women with normal pregnancy during this period were selected and classified as the control group.Retrospective analysis of the clinical significance of serum homocysteine,VB6,VB12,and folic acid levels in the two groups of patients during early pregnancy.Results:Except for the tHcy level,the other three levels in the comparison observation group were significantly lower than those in the control group.Based on the analysis of glycosylated hemoglobin in the observation group,the group was divided into a unsatisfactory blood sugar control group(G1)and a well blood sugar control group(G2)(Regardless of whether insulin or drugs control blood sugar),the folic acid level of the G1 group was significantly lower than that of the G2 group.The VB6 and VB12 levels of the G1 group were not statistically different from those of the G2 group.The tHcy level of the G1 group was significantly higher than that of the G2 group.Conclusion:serum homocysteine and vitamin content are closely related to the development process of GDM and pregnancy outcome.Vitamin supplementation during early pregnancy,especially folic acid supplementation,can improve the clinical prognosis of patients with gestational diabetes,thereby reducing the abnormal fetal development caused by diabetes and the short-term and long-term complications of offspring.
