经阴道三维超声微泡造影与增强磁共振对胎盘前置状态中孕引产患者胎盘植入诊断价值研究 被引量:13

Comparison of the value transvaginal three-dimensional ultrasound microvesicular imaging with that of enhanced magnetic resonance in the diagnosis of placenta implantation in pregnant patients with placenta previa requiring induced labor

摘要 目的探讨经阴道三维超声微泡造影与增强磁共振检查对胎盘前置状态中孕引产孕妇胎盘植入的诊断价值。方法选取2018年1月至2019年12月河南省人民医院48例中孕胎盘前置状态且要求引产的孕妇,均行经阴道三维超声微泡造影检查、增强磁共振检查。比较两种检查方法对中孕胎盘前置状态合并胎盘植入诊断的灵敏度、特异度、假阳性率(误诊率)、假阴性率(漏诊率)、准确率。结果 48例胎盘前置状态孕妇病理确诊胎盘植入者19例。经阴道三维超声微泡造影检查与增强磁共振检查的灵敏度、假阴性率及准确率,分别为89.47%vs.964.73%,10.53%vs. 5.26%,93.75%vs. 95.83%;两种检查方法的特异度和假阳性率均为96.56%和3.45%,差异无统计学意义。其中K值为0.832,说明两种检查方式诊断胎盘植入具有良好的一致性。结论经阴道三维超声微泡造影检查与增强磁共振检查对胎盘植入诊断的准确率均较高,可有效预测和评估胎盘植入的情况,指导引产方式的选择,降低引产风险。 Objective To investigate the diagnostic value of transvaginal three-dimensional ultrasound microvesicular contrast and enhanced magnetic resonance examination for placenta implantation in patients with placenta previa requiring induced labor.Methods A total of 48 pregnant women with placenta previa requiring induced labor in the middle of pregnancy who were admitted to Henan People’s Hospital from January 2018 to December 2019 were selected.Each patient underwent transvaginal three-dimensional ultrasound microvesicular imaging and enhanced magnetic resonance examination.The sensitivity,specificity,false positive rate(misdiagnosis rate),false negative rate(missed diagnosis rate) and accuracy of the two methods for the diagnosis of placenta previa in middle pregnancy with placenta implantation were compared.Results Among the 48 patients with placenta previa,there were 19 cases of placenta implantation confirmed by pathology.The sensitivity,false negative rate and accuracy of transvaginal 3 D ultrasound microvesicular imaging and enhanced magnetic resonance examination were 89.47% vs.964.73%,10.53% vs.5.26%,and 93.75% vs.95.83%,respectively.The specificity and false positive rate of the two methods were 96.56% and 3.45%,and the difference was not statistically significant.K value was 0.832,indicating that the two examination methods had a good consistency in the diagnosis of placenta implantation.Conclusion Both transvaginal three-dimensional ultrasound microvesicular imaging and enhanced magnetic resonance imaging have high accuracy in the diagnosis of placenta implantation,which can effectively predict and evaluate the situation of placenta implantation,guide the selection of labor induction methods,and reduce the risk of labor induction.
