多系统萎缩-C型患者运动皮质特征与病情严重程度相关性研究

摘要 目的观察多系统萎缩小脑型(MSA-C)患者运动诱发电位(MEP)与健康人群的差异,并探讨MSA患者MEP参数与共济失调严重程度相关性。方法选取2019年2月至2019年11月首都医科大学宣武医院神经内科住院的14例MSA患者为MSA组,社区招募15例与之年龄、性别相匹配的健康被试组成正常对照组。采用经颅磁刺激MEP作为观察指标,比较两组MEP潜伏期、时长、波形的变化及MSA患者MEP参数与病情严重程度的关系。结果MSA患者组MEP的潜伏期相比于健康对照组明显延长(P<0.01),MEP波形在MSA患者组及健康对照组有明显差异(P<0.01)。MSA患者MEP潜伏期与其共济失调评分成正相关(P<0.05)。结论MSA患者MEP潜伏期及波形与健康人群存在差异,MSA患者随着病情加重其MEP潜伏期逐渐延长,这对于协助评估MSA患者病情变化有一定帮助。MSA患者的MEP波形评级与共济失调评分有无相关性有待进一步研究。 Objective To observe the difference of motor evoked potentials(MEP)between patients with multiple system atrophy cerebellar types(MSA-C)and healthy people,and to explore the correlation between MEP parameters and the severity of ataxia in patients with MSA.Methods 14 patients with MSA in Xuanwu Hospital from February 2019 to November 2019 were selected as the MSA group,and 15 healthy subjects matched with age and sex were selected as the control group.MEP was used as an observation index to compare the changes of latency,duration and waveform between the two groups.Furthermore,we studied the relationship between MEP parameters and disease severity in patients with MSA.Results The latency of MEP in the MSA group was longer than that in the control group(P<0.01).There was a significant difference in the MEP waveform between MSA group and control group(P<0.01).The latency of MEP in patients with MSA was positively correlated with the scores of ataxia.Conclusion The latency and waveform of MEP in patients with MSA are different from those in healthy people.With the aggravation of the disease,the latency of MEP in patients with MSA is gradually prolonged,which is helpful in evaluating the clinical development of MSA.It needs to be further studied whether there is a correlation between MEP waveform rating and the scores of ataxia in patients with MSA.
