摘要
目的:分析基于健康管理平台在慢性肾脏病患者自我管理中的应用价值。方法:选取2019年1月-2020年6月已出院的慢性肾脏病患者100例,通过出院先后顺序分为两组,各50例。对照组提供常规健康管理模式;研究组建立慢性肾脏病健康管理平台,并提供相应的平台使用方法指导。比较两组患者生活质量评分、遵医行为评分、自我管理效果及疾病认知度。结果:研究组患者生活质量评分、遵医行为评分、自我管理效果及疾病认知度均优于对照组,差异有统计学意义(P<0.05)。结论:针对出院的慢性肾脏病患者,建立慢性肾脏病健康管理平台,并提供相应的平台使用方法指导工作,可将患者生活质量评分、遵医行为评分、自我管理效果以及疾病认知度优化至最佳状态。
Objective:To analyze the applied value of self-management based on health management platform in patients with chronic kidney disease.Methods:From January 2019 to June 2020,100 cases of discharged patients with chronic kidney disease were enrolled,they were divided into two groups according to discharge sequence,with 50 cases in each groups.The control group provided conventional health management model.The study group established a health management platform for chronic kidney disease and provided guidance on the use of the platform.The quality of life score,treatment compliance behavior score,self-management effect and disease recognition degree were compared between the two groups.Results:The quality of life score,treatment compliance behavior score,self-management effect and disease recognition degree of patients in the study group were significantly better than those in the control group,the differences were statistically significant(P<0.05).Conclusion:To establish a health management platform for chronic kidney disease patients who discharged from hospital and to provide guidance for the use of the platform,the quality of life score,treatment compliance behavior score,self-management effect and disease recognition degree of patients can be optimized to the best state.
