摘要
目的:探讨腹腔镜联合钬激光对胆总管结石患者T管放置率及残余结石率的影响。方法:选取胆总管结石患者80例,采用随机数字表法分为两组,每组各40例。对照组采用腹腔镜常规胆总管探查术,观察组采用腹腔镜联合钬激光碎石术,比较两组手术指标、T管放置率、残余结石率、复发率及并发症发生情况。结果:观察组手术时间、取石时间、住院时间均低于对照组,差异有统计学意义(P<0.05);与对照组比较,观察组结石残余率、复发率、T管放置率均较低,差异有统计学意义(P<0.05);观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:胆总管结石患者采用腹腔镜联合钬激光治疗的效果较好,可降低T管放置率、残余结石率,减少术后复发及并发症的发生。
Objective:To investigate the effects of laparoscopy combined with holmium laser on T-tube placement rate and residual stone rate in patients with choledocholithiasis. Methods: 80 patients with choledocholithiasis were randomized into two groups, with 40 cases in each group. The control group were treated with conventional laparoscopic common bile duct exploration, while the observation group were treated with laparoscopic combined with holmium laser lithotripsy. The operation indexes, T-tube placement rate, residual stone rate, recurrence rate and complications were compared between the two groups. Results: The operation time, stone extraction time and hospitalization time in the observation group were lower than those in the control group, the difference was statistically significant(P<0.05);compared with the control group, the residual rate, recurrence rate and T tube placement rate of the observation group were lower, the difference was statistically significant(P<0.05);the incidence of postoperative complications in the observation group was lower than that in the control group, the difference was statistically significant(P<0.05). Conclusion: Laparoscopic combined with holmium laser in the treatment of choledocholithiasis has a good effect, which can lower the T tube placement rate, residual stone rate, and reduce postoperative recurrence and complications in patients.
