侧卧位牵引结合术中正侧位透视行PFNA治疗老年股骨转子间骨折的临床研究

摘要 目的研究分析侧卧位牵引结合术中正侧位透视行PFNA治疗老年股骨转子间骨折的临床应用效果。方法对2017年7月-2020年7月收治的40例老年股骨转子间骨折患者进行回顾性分析,分为A、B两组。A组:牵引床侧卧位牵引,行PFNA髓内固定治疗,术中正位、侧位透视;B组:牵引床仰卧位牵引,行PFNA髓内固定治疗,术中正位、侧位透视。比较两组摆放体位时间、手术主切口长度、手术时间、术中出血量、骨折愈合时间、髋关节功能Harris评分的情况。结果40例患者均获随访,随访时间为2~26个月,平均13个月。两组在摆放体位时间、手术主切口长度、手术时间、术中出血量方面有明显差异(P<0.05),A组小于B组。A组和B组在骨折愈合时间、髋关节功能Harris评分方面无明显差异(P>0.05)。结论侧卧位牵引结合术中正侧位透视使PFNA治疗老年股骨转子间骨折手术操作更加简单易于掌握、置钉更加准确,创伤更小,手术时间更短,出血量更少,值得临床推广应用。 Objective To research the clinical effect of the lateral recumbent position traction with fore and anterolateral fluoroscopy during opreation on fixation of eldely femoral intertrochanteric fracture using proximal femoral nail antirotation(PFNA).Methods A retrospective analysis was performed on 40 patients of eldely femoral intertrochanteric fracture from July 2017 to July 2020.They were divided into two group.A group was treated with the proximal femoral nail antirotation(PFNA),in the lateral recumbent position traction with fore and anterolateral fluoroscopy during opreation.B group was treated with the proximal femoral nail antirotation(PFNA),in the supine position traction with fore and anterolateral fluoroscopy during opreation.The position setting time、length of main incision、operative time、the blood-loss in operation、the fracture healing time and the function of hip joint according to the Harris score system were analyzed and compared.Results All 40 cases were followed up from 2 months to 26 months,(average of 13 months).The position setting time、length of main incision、operative time、the blood-loss in operation were less in group A,and there were significant differences in the comparation.The fracture healing time and the function of hip joint according to the Harris score system were similar between A and B.Conclusion The lateral recunbent position traction with fore and anterolateral fluoroscopy during opreation in fixation of eldely femoral intertrochanteric fracture using proximal femoral nail antirotation(PFNA)has the advantages of simple procedure,less trauma.Shorter operation time,and less bleeding,which is worthy of promotion.
