摘要
目的评价3D打印模板引导^(125)碘(^(125)I)粒子植入治疗肺转移瘤的临床近期疗效。方法基于病历资料记录,对2019年3-12月于新疆医科大学附属肿瘤医院行放射性粒子植入的17例肺转移瘤患者,共26个转移病灶行回顾性分析,处方剂量150 Gy,粒子活度22.2~29.6 MBq。粒子植入术后2月、4月、6月行肺部CT扫描及近距离治疗计划系统剂量验证,评价肿瘤局部控制率、不良反应和粒子植入质量。结果术后2个月复查胸部CT,26个转移灶中达CR17个(65.4%),PR 5个(19.2%),SD4个(15.4%),客观缓解率(CR+PR)为84.6%(22/26),局部控制率(CR+PR+SD)为100%(26/26)。术后即刻验证与术后2月剂量验证D90差异无统计学意义(t=-1.276,P=0.214)。术后即刻粒子分布质量与术后2月粒子分布质量差异有统计学意义(χ^(2)=4.952,P=0.026)。结论3D打印模板引导粒子植入治疗肺转移瘤局部疗效明显,术后动态剂量验证提示剂量分布变化,值得进一步研究。
Objective To investigate the short-term effect of radioactive^(125)I seed implantation on pulmonary metastases assisted by 3D printing template.Methods A total of 17 patients with 26 lesions of pulmonary metastases in the hospital from March 2019 to December 2019 were enrolled in this retrospective study.Seed activity was 22.2-29.6 MBq and the prescription dose was 150 Gy.All patients were followed up for 2,4 and 6 months after the operation.The clinical efficacy,adverse reactions and implantation quality were evaluated by CT scan and BTPS(brachytherapy treatment planning system).Results Of the 26 pulmonary metastases,at 2 months of post-operation,17 achieved CR(65.4%),5 achieved PR(19.2%),and 4 achieved SD(15.4%).The objective response rate was 84.6%(22/26),while the local control rate was 100%(26/26).The difference in D90(the dose received by 90%target volume)between immediate and 2 months of post-operation was not statistically significant(t=-1.276,P=0.214).The difference in the implantation quality between immediate and 2 months of post-operation was statistically significant(χ^(2)=4.952,P=0.026).Conclusion 3D printing template assisted radioactive^(125)I seed implantation procedure possesses satisfying clinical effects in treating pulmonary metastases with minimal damage and few complications.The dynamic dose verification after operation indicates the changes of dose distribution,which needs further research.
