摘要
目的探讨检测下肢骨折术后患者血清中D-二聚体(D-D)、纤维蛋白单体(FM)及血栓调节蛋白(TM)的价值。方法180例下肢骨折手术患者,依据术后是否出现下肢深静脉血栓(DVT)将患者分为观察组(发生DVT,44例)及对照组(未发生DVT,136例)。测定所有患者的D-D、FM、TM,比较两组D-D、FM、TM,观察联合测定对DVT的诊断效能。结果观察组的D-D、FM、TM分别为(13.35±8.52)μg/ml、(22.29±10.35)μg/ml、(9.28±2.55)IU/ml,均高于对照组的(6.63±3.17)μg/ml、(9.46±4.55)μg/ml、(6.30±1.17)IU/ml,差异均具有统计学意义(P<0.05)。联合测定对DVT的诊断敏感度、特异度及准确率均高于单独测定,差异具有统计学意义(P<0.05)。结论下肢骨折术后发生DVT的患者D-D、FM、TM表达增高,测定D-D、FM、TM可应用于DVT防治,联合测定D-D、FM、TM可提高对DVT的诊断效能。
Objective To discuss the value of D-Dimer(D-D), fibrin monomer(FM) and thronmbomodulin in serum of patients with lower limb fracture after surgery. Methods A total of 180 patients with lower limb fracture were divided into observation group(with DVT, 44 cases) and control group [without deep venous thrombosis(DVT), 136 cases] according to the occurrence of DVT. The D-D, FM and TM of all patients were detected. The D-D, FM and TM were compared between the two groups, and the diagnostic efficacy of the combined detection for DVT was observed. Results The D-D, FM and TM of the observation group were(13.35± 8.52) μg/ml,(22.29±10.35) μg/ml and(9.28±2.55) IU/ml, which were all higher than(6.63±3.17) μg/ml,(9.46±4.55) μg/ml and(6.30±1.17) IU/ml of the control group, and the difference was statistically significant(P<0.05). The sensitivity, specificity and accuracy of combined detection for DVT were higher than those of single detection, and the difference was statistically significant(P<0.05). Conclusion The expressions of D-D, FM, and TM are increased in patients with DVT after lower limb fractures. The detection of D-D, FM, and TM can be used in the prevention and treatment of DVT, and the combined detection of D-D, FM, and TM can improve the diagnostic efficiency of DVT.
