摘要
目的:分析血液分析仪测定血红蛋白(Hb)的结果受到乳糜血样的影响情况。方法:选取本院于2018年5月~2019年7月接收的105例健康体检者为研究对象,将其随机分为Ⅰ组、Ⅱ组、Ⅲ组,采集其血液标本后,将其中的血浆分别置换为25、50、75μL的等量脂肪乳,用来模拟乳糜标本。观察并分析血液分析仪测定Hb的结果受到脂肪乳的影响情况。结果:将不同量的脂肪乳加入到受检者的血液标本中以后,均引起了Hb假性增高,也即Ⅰ组、Ⅱ组、Ⅲ组置换脂肪乳后测得的Hb值要明显高于直接测定的值,且置换后增加的比分为3.81%、6.47%、10.78%,差异明显,P<0.05,具有统计学意义。结论:采用血液分析仪对乳糜血样进行测定时,会出现Hb假性增高的情况,并且对Hb的影响会随着脂肪乳含量的增加而不断增大。为确保Hb测定结果的准确性,可考虑采用生理盐水置换血浆,以消除脂浊造成的影响。
Objective: To analyze the effect of chylous blood samples on the determination of hemoglobin(Hb) by the blood analyzer. Methods: The 105 health checkups received by this hospital from May 2018 to July 2019 were selected as the research objects, and they were randomly divided into groups Ⅰ, Ⅱ, and Ⅲ. After collecting blood samples, the plasma replace with 25, 50, 75μL of the same amount of fat emulsion to simulate chyle specimens. Observe and analyze the influence of the fat emulsion on the Hb result of the blood analyzer. Results: Adding different amount of fat emulsion to the client after blood samples, have caused a higher Hb pseudo, namely Ⅰ group, Ⅱ group, Ⅲ replacement of fat emulsion of Hb values are significantly higher than the direct determination of value, and the displacement increase after the score was 3.81%, 6.47%, 10.78%, significant difference, P<0.05, with statistical significance. Conclusion: When chylous blood sample was measured by blood analyzer, false increase of Hb would occur, and the influence on Hb would increase with the increase of fat and milk content. In order to ensure the accuracy of Hb test results, the use of normal saline to replace plasma can be considered to eliminate the influence of lipid turbidity.
