神经病变和非神经病变型2型糖尿病患者的电生理变化及与TNF-α的相关性研究 被引量:1

Research on the correlation of electrophysiological changes and TNF-αin patients with type 2 diabetes mellitus of diabetic neuropathy and non-diabetic peripheral neuropathy

摘要 目的探讨2型糖尿病非神经病变(NDPN)及神经病变(DN)患者的电生理变化及与血清TNF-α的相关关系。方法选取2018年7月至2020年7月于安徽省立医院确诊的86例2型糖尿病患者,根据有无DN分为DN组(n=43)与NDPN组(n=43)。每组均检测血清肿瘤坏死因子-α(TNF-α)、正中神经、腓总神经的感觉神经传导速度(SNCV)、感觉神经动作电位(SNAP)波幅及运动神经的传导速度(MNCV),评价DN患者SNCV、SNAP波幅及MNCV与TNF-α的相关性。结果与NDPN组相比,DN组中正中神经SNCV、SNAP波幅、MNCV下降,腓总神经SNCV、MNCV下降,TNF-α含量升高(P均<0.05),而腓总神经SNAP波幅变化不显著(P>0.05)。DN组正中神经SNCV、SNAP波幅、MNCV及腓总神经SNCV、MNCV与TNF-α呈负相关,而腓总神经SNAP波幅变化与TNF-α无相关关系。结论T2DM患者血清TNF-α水平是DN发生的重要影响因素,在一定程度上可以用作临床早期诊断糖尿病神经病变的重要参考指标。 Objective To investigate the correlation of electrophysiological changes and serum TNF-αin patients with type 2 diabetes mellitus(T2DM)of non-diabetic peripheral neuropathy(NDPN)and diabetic neuropathy(DN).Methods A total of 86 patients with T2DM diagnosed in Anhui Provincial Hospital from July 2018 to July 2020 were divided into the DN group(n=43)and the NDPN group(n=43)according to the presence or absence of DN.Serum tumor necrosis factor-α(TNF-α),sensory nerve conduction velocity(SNCV),sensory nerve action potential(SNAP)amplitude and motor nerve conduction velocity(MNCV)of median nerve and common peroneal nerve were detected in each group.Meanwhile,the correlation among SNCV,SNAP amplitude and MNCV with TNF-αin patients with DN was evaluated.Results Compared with the NDPN group,SNCV,SNAP amplitude and MNCV of median nerve were decreased,SNCV and MNCV of common peroneal nerve were decreased,and TNF-αlevel was increased in the DN group(all P<0.05).Meanwhile,there was no significant change in SNAP amplitude of common peroneal nerve(P>0.05).In the DN group,SNCV,SNAP amplitude,MNCV of median nerve,and SNCV,MNCV of common peroneal nerve were negatively correlated with TNF-α,while there was no correlation between SNAP amplitude of common peroneal nerve and TNF-α.Conclusion The level of serum TNF-αin patients with T2DM is an important factor impacting the occurrence of DN,which can be used as an important reference index for early clinical diagnosis of diabetic neuropathy.
