摘要
目的通过超声监测麻醉诱导时不同通气模式下胃胀气发生情况,探讨胃窦超声在麻醉评估中的临床意义。方法将160例行腹腔镜胆囊切除术的全身麻醉患者随机分为M组和P组,每组80例。给予两组不同的面罩通气方式及麻醉方式。超声测量并记录两组面罩通气前后胃胀气发生情况及胃窦部横截面积(CSA)。结果M组四种面罩通气模式下胃胀气发生率差异显著(P<0.05),其中压控20 cmH2O>压控15 cmH2O>压控10 cmH2O>手控通气。P组四种面罩通气模式下胃胀气发生率差异显著(P<0.05),其中压控20 cmH2O>压控15 cmH2O>压控10 cmH2O=手控通气。患者面罩通气后的平均胃窦部CSA高于面罩通气前(P<0.05)。M组压控15 cmH2O面罩通气前后平均胃窦CSA变化值低于P组(P<0.05)。M组及P组胃窦部CSA变化组内比较皆有显著差异(P<0.05)。结论是否使用肌松剂对胃胀气无明显影响;手控通气模式下胃胀气发生率明显低于压控通气,胃胀气发生率随着压控目标压力的增高呈增加的趋势。
Objective To investigate the clinical significance of gastric antrum ultrasonography in anesthesia evaluation by monitoring the occurrence of flatulence under different ventilation modes during anesthesia induction.Methods A total of 160 patients undergoing laparoscopic cholecystectomy under general anesthesia were randomly divided into group M and group P,with 80 cases in each group.The two groups were given different ways of mask ventilation and anesthesia.The incidence flatulence and the cross-sectional area(CSA)of gastric antrum were measured and recorded before and after mask ventilation.Results There was significant difference in the incidence of flatulence among the four mask ventilation modes in the group M(P<0.05),in which pressure controlled 20 cmH2O>pressure controlled 15 cmH2O>pressure controlled 10 cmH2O>manual ventilation.There was significant difference in the incidence of flatulence among the four mask ventilation modes in the group P(P<0.05),in which pressure controlled 20 cmH2O>pressure controlled 15 cmH2O>pressure controlled 10 cmH2O=manual ventilation.The mean CSA of gastric antrum after mask ventilation was higher than that before mask ventilation(P<0.05).The mean changes value of gastric antrum CSA before and after pressure controlled 15 cmH2O mask ventilation in the group M was lower than that in the group P(P<0.05).The changes of gastric antrum CSA in the group M and the group P were significantly different(P<0.05).Conclusion The use or no of muscle relaxant has no obvious effect on flatulence;the incidence of flatulence in manual ventilation mode is significantly lower than that in pressure ventilation mode,and the incidence of flatulence tended to increase with the increase of the target pressure of pressure controlled.
