摘要
目的探讨妊娠滋养细胞肿瘤患者化疗预后的影响因素。方法选择2016年1月至2019年12月在本院接受化疗的妊娠滋养细胞肿瘤患者60例,根据化疗预后情况不同进行分组,化疗预后较差的患者作为观察组(n=11),化疗预后较好的患者作为对照组(n=49),分析妊娠滋养细胞肿瘤患者化疗预后的影响因素。结果两组年龄、前次妊娠性质等情况比较,差异无统计学意义(P>0.05);观察组距前次妊娠间隔时长≥7个月、病灶大小≥5cm、转移病灶数目≥5个、临床分期Ⅲ~Ⅳ期、化疗前血HCG水平≥10000U/L、国际妇产科联盟(FIGO)预后评分≥7分的患者占比均高于对照组,差异有统计学意义(P<0.05);经Logistic多因素分析显示,距前次妊娠间隔时长≥7个月、病灶大小≥5cm、转移病灶数目≥5个、临床分期Ⅲ~Ⅳ期、化疗前血HCG水平≥10000U/L、FIGO预后评分≥7分是影响妊娠滋养细胞肿瘤患者化疗预后的危险因素(OR>1,P<0.05)。结论距前次妊娠间隔久、病灶大、转移病灶数目多、临床分期严重、化疗前血HCG水平高、FIGO预后评分高是影响妊娠滋养细胞肿瘤患者化疗预后的危险因素。
Objective To investigate the influencing factors of chemotherapy prognosis in patients with gestational trophoblastic tumor.Methods A total of 60 patients with gestational trophoblastic tumor who were admitted to our hospital for chemotherapy from January 2016 to December 2019 were selected and divided into two groups according to the status of prognosis.Patients with poor prognosis were classified as observation group(n=11),and patients with good prognosis of chemotherapy were classified as control group(n=49).The influencing factors of chemotherapy prognosis in patients with gestational trophoblastic tumor were analyzed.Results the influencing factors of chemotherapy prognosis in patients with gestational trophoblastic tumor There were no significant differences of age and nature of previous pregnancy,etc.between the two groups(P>0.05).The proportion of patients with duration of previous pregnancy ≥7 months,the lesion size≥5 cm,the number of metastatic lesions ≥5,clinical stage Ⅲ-Ⅳ,blood HCG level before chemotherapy ≥10000U/L,the International Federation of Obstetrics and Gynecology(FIGO)prognosis score ≥7 of the observation group were higher than the control group,the differences were statistically significant(P<0.05).Logistic multivariate analysis showed that the interval from the previous pregnancy ≥7 months,the size of the lesion ≥5cm,the number of metastatic lesions≥5,the clinical stage Ⅲ-Ⅳ,the blood HCG level before chemotherapy ≥10000U/L,and the FIGO prognosis score ≥7 points were the risk factors affected the prognosis of chemotherapy in patients with gestational trophoblastic tumor(OR>1,P<0.05).Conclusion Long time interval from previous pregnancy,large lesions,large number of metastatic lesions,high blood HCG levels before severe chemotherapy in clinical staging,and high FIGO prognosis score are risk factors that affect the prognosis of chemotherapy in patients with gestational trophoblastic tumors.
