摘要
目的探讨不同皮瓣修复术对面颈部瘢痕患者皮瓣存活率及再手术率的影响。方法选择本院皮肤科2017年11月至2019年11月收治的面颈部瘢痕患者35例,依据治疗方式不同分为对照组(n=17)与观察组(n=18),对照组患者予以传统皮瓣修复术,观察组患者则予以胸部扩张器皮瓣修复术。对比两组术前、术后3个月外形与功能评分及皮瓣存活率、再手术率。结果两组术前外形与功能评分对比,差异无统计学意义(P>0.05),术后3个月,两组外形与功能评分均上升,观察组上升幅度较大,差异有统计学意义(P<0.05);相比对照组,观察组术后3个月内皮瓣存活率较高,术后3个月内再手术率较低,差异有统计学意义(P<0.05)。结论面颈部瘢痕患者采用胸部扩张器皮瓣修复效果优于传统皮瓣修复,可有效提升外形与功能评分,提升皮瓣存活率,降低再手术率,值得临床推广。
Objective To investigate the effect of different flap repairs on flap survival rate and reoperation rate in patients with facial and neck scars.Methods A total of 35 patients with facial and neck scars who were treated in our hospital from November 2017 to November 2019 were selected and were divided into the control group(n=17)and the observation group(n=18)according to different treatment methods.Patients in the control group were treated with traditional flap repair,and patients in the observation group were treated with chest dilator flap repair.The appearance and function score,flap survival rate and reoperation rate of the two groups before and after operation were compared.Results There was no statistically significant difference in preoperative appearance and function score before operation between the two groups(P>0.05).At 3 months after operation,both appearance and function scores of the two groups increased,and the observation group had a larger increase,the difference was statistically significant(P<0.05).Compared with the control group,the endothelial flap survival rate was higher in the observation group at 3 months after operation,and the reoperation rate was lower,the differences were statistically significant(P<0.05).Conclusion The effect of chest dilator flaps repair to patients with facial and neck scars is better than the traditional flaps.It can effectively improve the appearance and function score,improve the survival rate of the flap,reduce the reoperation rate and it is worthy of clinical promotion.
