SF、CA125、CA15-3在恶性淋巴瘤骨髓受累中的诊断价值 被引量:3

摘要 目的探讨SF、CA125及CA15-3在恶性淋巴瘤骨髓受累中的诊断价值。方法选取2016年12月至2018年12月就诊于我院血液科的恶性淋巴瘤患者103例作为研究对象,其中41例骨髓受累患者为受累组,62例骨髓未受累患者为非受累组;采用Logistic回归分析筛选出恶性淋巴瘤骨髓受累的实验室标志物,建立标志物的预测模型,采用ROC曲线评价该模型。结果受累组患者CEA、CA125、CA15-3、SF水平均显著高于非受累组(P<0.05),而SCC、CA724、AFP、CA19-9、CYRA21-1、NSE比较,差异无统计学意义(P>0.05);进一步行多因素Logistic回归分析结果显示,CA125、CA15-3、SF水平与骨髓受累的相关性分别为1.033、1.152、1.005,差异均有统计学意义(P<0.05);最终恶性淋巴瘤骨髓受累的预测模型为P=1/[1+e(-12.382-8.027CA125+0.209CA15-3+6.004SF)],且预测模型的AUC为0.970,显著高于其他单一标志物的AUC。结论联合SF、CA125和CA15-3的预测模型有助于提高恶性淋巴瘤骨髓受累的诊断效能,具有临床应用价值。 Objective To explore the diagnostic value of SF,CA125 and CA15-3 in bone marrow involvement in malignant lymphoma.Methods A total of 103 patients with malignant lymphoma treated in Department of Hematology of our hospital from December 2016 to December 2018 were selected as research subjects,of which 41 patients with bone marrow involvement were divided into the involved group and 62 patients without bone marrow involvement were divided into the non-involved group.Logistic regression analysis was used to screen laboratory markers of bone marrow involvement in malignant lymphoma,and a prediction model of the markers was established,which was then evaluated through ROC curve.Results The levels of CEA,CA125,CA15-3 and SF in theinvolved group were all significantly higher than those in the non-involved group(P<0.05),while there was no statistically significant difference between the two groups in SCC,CA724,AFP,CA19-9,CYRA21-1 and NSE(P>0.05).Further multivariate Logistic regression analysis showed that the correlation between CA125,CA15-3 and SF levels and bone marrow involvement was 1.033,1.152 and 1.005 respectively,with statistical differences(P<0.05).Ultimately,the prediction model for bone marrow involvement in malignant lymphoma was P=1/[1+e(-12.382-8.027CA125+0.209CA15-3+6.004SF)],and the AUC of the prediction model was 0.970,which was significantly higher than that of other single markers.Conclusion The predictive model combining SF,CA125 and CA15-3 helps improving the diagnostic efficiency of bone marrow involvement in malignant lymphoma.It is of significance for clinical application.
