摘要
目的研究维持性血液透析患者血红蛋白(Hb)、血清白蛋白(Alb)、C反应蛋白(CRP)与感染的相关性及预后死亡的危险因素。方法选取本院2018年4月—2020年4月收治的经临床确诊为终末期肾脏病并行维持性血液透析的88例为研究对象。根据有无感染分为感染组36例和非感染组52例。比较2组CRP、Alb及Hb水平,并分析影响预后死亡的危险因素。结果感染组CRP水平显著高于非感染组,Alb及Hb水平显著低于非感染组(P<0.01)。感染组病死率高于非感染组(P<0.05)。年龄≥60岁、合并糖尿病、合并高血压、合并冠心病、合并感染、Alb<35 g/L及Hb<110 g/L是影响维持性血液透析患者预后死亡的独立危险因素(P<0.05,P<0.01)。结论维持性血液透析患者CRP、Alb及Hb水平与其感染率具有一定相关性;且通过熟悉掌握影响预后死亡的危险因素,积极采取相应措施,可提高患者长期存活率。
Objective To study the correlation of hemoglobin(Hb),serum albumin(Alb),and C-reactive protein(CRP)with infection in patients undergoing maintenance hemodialysis and to analyze the risk factors of death.Methods A total of 88 patients who were admitted to our hospital from April 2018 to April 2020 and were clinically diagnosed with end-stage renal disease(ESRD)and underwent maintenance hemodialysis were selected as the research subjects.They were divided into infection group(n=36)and non-infection group(n=52)according to presence of infection.The levels of CRP,Alb and hemoglobin were compared between the two groups,and the risk factors of death were analyzed.Results CRP level of infection group was significantly higher than that of non-infection group,while Alb and Hb levels were significantly lower than those of non-infection group(P<0.05).Mortality of infection group was higher than that of non-infection group(P<0.05).Age≥60 years,combined diabetes,hypertension,coronary heart disease,infection,Alb<35 g/L and Hb<110 g/L were the independent risk factors of death of patients undergoing maintenance hemodialysis(P<0.05,P<0.01).Conclusion CRP,Alb and Hb levels of patients undergoing maintenance hemodialysis have certain correlation with their infection rates.By familiarizing themselves with the risk factors that affect the death,and actively taking corresponding measures can the long-term survival rate of patients be improved..
