摘要
目的比较直接前侧入路(DAA)与后外侧入路(PA)行全髋关节置换术(THA)的疗效。方法将60例行THA患者按随机数字表法分为PA组和DAA组,每组30例。比较两组切口长度、术中出血量、手术时间、住院时间、下床时间、部分负重锻炼时间和术后并发症情况。术后6、12个月采用髋关节Harris评分标准评价髋关节功能。结果患者均获得随访,时间12~18个月。手术时间DAA组明显长于PA组(P<0.001);切口长度、术中出血量、住院时间、下床时间、部分负重锻炼时间DAA组明显短(少)于PA组(P<0.001)。术后并发症发生率DAA组明显低于PA组(P<0.05)。两组均无深静脉血栓形成、假体松动、髋关节脱位等严重并发症发生。髋关节功能优良率:术后6个月DAA组明显高于PA组(P<0.05);术后12个月两组比较差异无统计意义(P>0.05)。结论与PA相比,采用DAA行THA可有效减少手术创伤及并发症的发生,更利于患者早期锻炼和髋关节功能恢复。
Objective To compare the efficacy of total hip arthroplasty(THA)with direct anterior approach(DAA)and posterolateral approach(PA).Methods The 60 patients treated with THA were divided into PA group and DAA group according to the random number table method,there were 30 cases in each group.The incision length,intraoperative blood loss,operation time,hospitalization time,getting out of bed time,partial weight-bearing exercise time and postoperative complications were compared between the two groups,and the hip Harris function scores criterion were performed at 6,12 months after operation.Results The 60 patients were followed up for 12~18 months.The operation time of DAA group was significantly longer than that of PA group(P<0.001);the incision length,intraoperative blood loss and the hospitalization time,time of getting out of bed,partial weight-bearing exercise of DAA group were significantly shorter(less)than those of PA group(P<0.001).The complication incidence rate of DAA group was significantly lower than that of PA group(P<0.05),and there were no severe complications such as deep venous thrombosis,prosthesis loosening,dislocation of hip joint in the two groups.The hip functional excellent-good rate of DAA group was significantly higher than that of PA group at 6 months after operation(P<0.05),and there was no statistical significance between the two groups at 12 months postoperation(P>0.05).Conclusions Compared with PA,the THA with DAA can effectively reduce the surgical trauma and complications of patients,which is helpful to the early exercise and the hip joint function recovery.
