摘要
目的:观察符合阴道分娩的足月单胎妊娠初产妇在活跃期停滞时应用中医穴位按摩联合地西泮治疗的临床效果。方法:选取2017年11月-2019年11月本院产科活跃期停滞的符合阴道分娩的足月单胎妊娠初产妇66例。随机分为对照组33例和观察组33例。对照组予精神、心理支持,指导产妇合理运用呼吸方法,开放静脉通路,给予静脉支持,观察组在此基础上增加穴位按摩与地西泮联合治疗。观察两组产妇产程时间、宫颈成熟率、产后24 h出血量、不良分娩结局(胎儿窘迫及新生儿窒息)发生率。结果:观察组第一产程、第二产程时间、产后24 h出血量均优于对照组,差异有统计学意义(t=6.185、5.253、5.457,P<0.05)。观察组不良分娩结局(胎儿窘迫、新生儿窒息)发生率为6.1%,低于对照组的24.2%,差异有统计学意义(χ^2=4.379,P=0.026)。观察组宫颈总成熟率为90.91%,高于对照组的66.67%,差异有统计学意义(χ^2=7.469,P=0.004)。结论:对活跃期停滞的产妇在产程处理时使用中医穴位按摩联合地西泮的方法,可有效缓解宫颈水肿、痉挛,软化宫颈,加快宫颈成熟及宫口扩张,缩短产程,减少产后出血量,降低分娩过程中胎儿窘迫及新生儿窒息的发生率。
Objective:To observe the clinical efficacy of traditional Chinese medicine acupoint massage combined with Diazepam in the treatment of full-term single pregnancy primipara in accordance with vaginal delivery during arrested active phase.Method:A total of 66 primiparas with full-term singleton pregnancy who were in accordance with vaginal delivery from November 2017 to November 2019 in our hospital were selected.They were randomly divided into the control group (33 cases) and the observation group (33 cases).The control group was given mental and psychological support,guided the puerpera to use breathing method reasonably,opened venous access and given intravenous support,while the observation group added acupoint massage combined with Diazepam on this basis.The birth process time,cervical ripening rate,blood loss 24 h after delivery,adverse delivery outcomes (fetal distress and neonatal asphyxia) were observed.Result:The time of the first stage of labor,the second stage of labor and the amount of postpartum hemorrhage in the observation group were significantly better than those in the control group,and the differences were statistically significant (t=6.185,5.253,5.457,P<0.05).The incidence of adverse delivery outcomes (fetal distress and neonatal asphyxia) in the observation group was 6.1%,which was lower than 24.2% in the control group,the difference was statistically significant (χ^2=4.379,P=0.026).The cervical ripening rate of the observation group was 90.91%,which was higher than 66.67% of the control group,the difference was statistically significant (χ^2=7.469,P=0.004).Conclusion:The application of traditional Chinese medicine acupoint massage combined with Diazepam can effectively relieve cervical edema and spasm,soften the cervix,accelerate cervical ripening and dilatation,shorten the delivery process,reduce the amount of postpartum hemorrhage,and reduce the incidence of fetal distress and neonatal asphyxia during delivery.
