摘要
目的:采用经侧裂入路显微手术治疗高血压脑出血患者,旨在对其血肿清除率及神经功能的影响进行探讨。方法:选取笔者所在医院2017年9月-2019年9月收治的高血压脑出血患者112例为研究对象,随机编号1~112号,将奇数号患者纳入A组(n=56),偶数号患者纳入B组(n=56)。A组采用经皮层造瘘入路,B组给予经侧裂入路显微手术,比较两组手术情况、血肿清除率、神经功能、格拉斯哥昏迷指数(GCS)评分及术后并发症。结果:B组手术时间及住院时间均短于A组,残余血量少于A组,差异均有统计学意义(P<0.05);B组血肿清除率优于A组,差异有统计学意义(P<0.05);治疗后1个月B组NIHSS评分低于A组,治疗后7 d B组GCS评分高于A组,差异均有统计学意义(P<0.05);B组并发症总发生率(3.57%)低于A组(14.29%),差异有统计学意义(P<0.05)。结论:高血压脑出血患者采用经侧裂入路显微手术治疗可有效缩短手术时长,减少残余血量,提高血肿清除率,改善患者昏迷状态及神经功能,减少术后并发症的发生与住院时长,有利于术后恢复。
Objective:To investigate the treatment of hypertensive intracerebral hemorrhage with microsurgery via lateral fissure approach,aiming to explore the effect of hematoma clearance rate and nerve function.Method:A total of 112 patients with hypertensive intracerebral hemorrhage treated in our hospital from September 2017 to September 2019 were selected as the research object,randomly numbered them from 1 to 112,patients with odd number were included in group A (n=56),patients with even number were included group B (n=56).Group A was treated with percutaneous fistula approach,and group B was given microsurgery via lateral fissure approach.The surgical conditions,hematoma clearance,nerve function,Glasgow coma index (GCS) score,and postoperative complications were compared between the two groups.Result:The operative time and hospital stay in group B were shorter than those in group A,and the residual blood volume was less than that in group A,the differences were statistically significant (P<0.05).The hematoma clearance rate in group B was better than that in group A,the difference was statistically significant (P<0.05).The NIHSS score 1 month after treatment in group B was lower than that in group A,and the GCS score 7 days after treatment in group B was higher than that in group A,the differences were statistically significant (P<0.05).The total incidence of complications in group B (3.57%) was lower than that in group A (14.29%),the difference was statistically significant (P<0.05).Conclusion:The treatment of hypertensive intracerebral hemorrhage with microsurgery via lateral fissure approach can effectively shorten the operation time,reduce the residual blood volume,improve the hematoma clearance rate,improve the coma state and nerve function,reduce the incidence of postoperative complications,and shorten the length of hospitalization.
