摘要
目的:探讨经皮椎间孔镜治疗腰椎间盘突出症的临床应用。方法:遵循随机数字表法将2017年3月-2019年4月潮州市人民医院收治的66例腰椎间盘突出症患者均分为对照组和观察组,各33例。其中对照组给予传统切开手术(腰椎间盘突出髓核切除术)治疗,观察组给予椎间孔镜下腰椎间盘突出髓核切除术治疗。比较分析两组治疗后的临床疗效、临床指标、腰椎功能、并发症发生情况、治疗前后的疼痛程度。结果:观察组的总有效率达96.97%,明显高于对照组,差异有统计学意义(P<0.05)。与对照组相比,观察组术中出血量明显更少,切口长度和术后住院时间明显更短,差异有统计学意义(P<0.05)。观察组术后腰椎功能优良率为84.85%,略高于对照组,但差异无统计学意义(P>0.05)。与对照组相比,观察组术后VAS评分明显更低,差异有统计学意义(P<0.05)。观察组术后并发症发生率为3.03%,明显低于对照组,差异有统计学意义(P<0.05)。结论:应用经皮椎间孔镜治疗腰椎间盘突出症效果较佳,提高患者临床指标,改善腰椎功能,显著降低术后疼痛程度和并发症,值得广泛推广。
Objective:To explore the clinical application of percutaneous intervertebral foramen endoscopic in the treatment of lumbar disc herniation.Method:According to the random number table method,66 patients with lumbar disc herniation admitted to Chaozhou People’s Hospital from March 2017 to April 2019 were divided into the control group and the observation group,33 cases in each group.The control group was given traditional incision surgery (lumbar disc herniation nucleus pulposus resection) treatment,the observation group was given intervertebral foramen endoscopic lumbar disc herniation nucleus pulpectomy treatment.The clinical efficacy,clinical indicators,lumbar function,complications and pain degree before and after treatment were compared and analyzed between the two groups.Result:The total effective rate of the observation group was 96.97%,which was significantly higher than that in the control group,the difference was statistically significant (P<0.05).Compared with the control group,the amount of bleeding in the observation group was significantly less,the incision length and postoperative hospital stay were significantly shorter,the differences were statistically significant (P<0.05).The excellent and good rate of lumbar function in the observation group was 84.85%,which was higher than that in the control group,but there was no significant difference (P>0.05).Compared with the control group,the VAS score of the observation group was significantly lower (P<0.05).The incidence of postoperative complications in the observation group was 3.03%,which was significantly lower than that in the control group,the difference was statistically significant (P<0.05).Conclusion:It is worth popularizing that the application of percutaneous intervertebral foramen endoscopic is better in the treatment of lumbar disc herniation,which can improve the clinical indicators,improve the lumbar function,significantly reduce the degree of postoperative pain and complications.
