摘要
目的:应用自行研发的专用漏血监测装置评估血液透析患者脱针率、漏血发生率和穿刺总满意度。方法:选取在医院进行血液透析的98例患者,按照随机数表法分将其为观察组和对照组,每组49例。对照组进行留置针穿刺,观察组在对照组的基础上应用自行研发的专用漏血检测装置,每例患者穿刺15次,比较两组穿刺成功率、脱针率、针头移位发生率、针刺伤发生率、漏血发生率、穿刺操作时间、止血时间等穿刺相关指标和渗血程度、总渗血以及患者对穿刺的满意度。结果:观察组在穿刺成功率、脱针率、针头移位发生率、针刺伤发生率以及漏血发生率明显优于对照组,两组比较差异有统计学意义(x^(2)=17.887,x^(2)=28.915,x^(2)=25.979,x^(2)=13.671,x^(2)=9.044;P<0.05);观察组穿刺操作时间和止血时间明显优于对照组,两组比较差异有统计学意义(t=24.897,t=38.397;P<0.05);观察组的轻度渗血、中度渗血、重度渗血发生率以及总渗血发生率明显优于对照组,两组比较差异有统计学意义(x^(2)=4.178,x^(2)=12.043,x^(2)=4.488,x^(2)=15.117;P<0.05);观察组的穿刺总满意度为96.19%,对照组穿刺总满意度为89.25%,两组比较差异有统计学意义(x^(2)=26.217,P<0.05)。结论:将专用漏血监测装置应用于血液透析治疗,可有效提高穿刺成功率、降低脱针率和渗血、漏血发生率,同时提高患者对穿刺的满意度。
Objective:To discuss the needle removal rate,the incidence of blood leakage and the total satisfaction of puncture in the treatment and application of the self-developed special blood leakage monitoring device.Methods:98 patients undergoing hemodialysis in the hospital were selected and divided into observation group(49 cases)and control group(49 cases)according to the random number table method.The control group underwent indwelling needle puncture,and the observation group applied a self-developed special blood leakage detection device on the basis of the control group.Each patient was punctured 15 times.The two groups were compared with the puncture success rate,needle removal rate,needle displacement incidence,the incidence of puncture wounds,the incidence of blood leakage,puncture operation time,hemostasis time and other puncture-related indicators,the degree of bleeding,total bleeding,and patient satisfaction with puncture.Results:The observation group was significantly better than the control group in puncture success rate,needle removal rate,needle displacement rate,needle stick injury rate,and blood leakage rate,the difference was statistically significant(x^(2)=17.887,x^(2)=28.915,x^(2)=25.979,x^(2)=13.671,x^(2)=9.044,P<0.05);the puncture operation time and hemostasis time in the observation group were significantly better than those in the control group,the difference was statistically significant(t=24.897,t=38.397;P<0.05);the incidence of light oozing,moderate oozing,severe oozing,and total oozing in the observation group were significantly better than those in the control group,the difference was statistically significant(x^(2)=4.178,x^(2)=12.043,x^(2)=4.488,x^(2)=15.117,P<0.05);the overall satisfaction rate of puncture in the observation group was 96.19%,which was higher than 89.25%in the control group,the difference was statistically significant(x^(2)=26.217,P<0.05).Conclusion:Applying a dedicated blood leakage monitoring device to hemodialysis treatment can effectively increase the success rate of
