摘要
目的:探讨HEOS冷刀宫腔镜微剪分离术对宫腔粘连切除的安全性及术后生育功能的影响。方法:随机选取医院收治的70例重度宫颈粘连患者,按照治疗方式的不同将其分为对照组和观察组,每组35例,对照组患者采用传统宫腔镜下电切分离术,观察组患者采用HEOS冷刀宫腔镜微剪分离术。对比两组患者临床疗效、手术情况、术后情况及随访情况,对比手术安全性和术后生育功能。结果:观察组患者临床疗效、月经改善率及妊娠率均高于对照组,差异具有统计学意义(x^(2)=6.437,x^(2)=5.510,x^(2)=5.922;P<0.05);观察组患者术后1个月、3个月及12个月再粘连发生率低于对照组,差异具有统计学意义(x^(2)=4.786,x^(2)=5.245,x^(2)=8.571;P<0.05);观察组手术时间、术中出血量及膨宫液吸收量均低于对照组,差异具有统计学意义(t=-10.939,t=-10.939,t=-10.939;P<0.05)。结论:对重度宫腔粘连患者采用HEOS冷刀宫腔镜微型剪刀宫腔粘连分离术治疗,可有效降低术中操作时间、术中出血量及膨宫液的吸收量,有效改善患者术后月经情况和妊娠率,降低术后宫腔再粘连的发生。
Objective:To investigate the effects of the exclusion with slight scissoring under hysteroscopy with HEOS cold knife on the safety of the excision of intrauterine adhesions and postoperative fertility function.Methods:Seventy patients with severe cervical adhesions admitted to hospital were randomly selected.And they were divided into control group and observation group according to different therapy method,with 35 cases in each group.The control group adopted traditional exclusion with electric incision under hysteroscopy,and observation group adopted the exclusion with slight scissoring under hysteroscopy with HEOS cold knife.The clinical efficacy,operation condition,postoperative condition and follow-up condition between the two groups were compared.And the safety of the operation and postoperative fertility function between two groups also were compared.Results:The clinical efficacy,menstrual improvement rate and pregnancy rate of observation group were significantly higher than those of control group(x^(2)=6.437,x^(2)=5.510,x^(2)=5.922,P<0.05),respectively.The incidences of re-adhesion of observation group at 1th,3th,and 12th months after operation were significantly lower than those of control group(x^(2)=4.786,x^(2)=5.245,x^(2)=8.571,P<0.05),respectively.The operation time,the intraoperative bleeding amount,and the absorption amount of the fluid of dilating uterine of observation group were significantly lower than those of control group(t=-10.939,t=-10.939,t=-10.939,P<0.05),respectively.Conclusion:For patients with severe intrauterine adhesions,the used the exclusion with mini-type scissor under hysteroscopy with HEOS cold knife for intrauterine adhesion can effectively reduce intraoperative operation time,the intraoperative bleeding amount and the absorptive amount of the fluid of dilating uterine,and can effectively improve postoperative menstruation condition and pregnancy rate,and can reduce the incidence of postoperative re-adhesion of uterine.
