摘要
目的探讨处方前置审核干预对某院质子泵抑制剂使用的影响。方法选取2019年1月~6月在围术期使用质子泵抑制剂的200份病历作为对照组;另选取2019年7月~12月在围术期使用质子泵抑制剂的200份病历为观察组,此期间对病历医嘱进行处方前置审核并对不合理医嘱给医生作出提示修改,通过药师对系统筛选出来的问题医嘱结合病历进行再审核及点评,结合行政辅助管理措施进行改进,从预防性用药情况、使用强度、联合用药不当、疗程过长、用法用量不合理、给药途径不适宜六个方面评价两组病历的合理性。结果经过处方前置审核+行政管理辅助措施,不合理用药现象显著减少(从78%下降至19.5%),无高危因素预防性用药情况得到改善(从81.92%下降至31.98%)、使用强度显著降低。结论处方前置审核干预可有效促进围术期质子泵抑制剂合理使用。
Objective The article aims to investigate the performing effect of prescription pre-audit intervention on proton pump inhibitors(PPIs)in a hospital.Methods A total of 200 medical records applying PPIs during perioperative period from January to June 2019 were selected as the control group.Another 200 medical records using PPIs during perioperative period from July to December 2019 were selected as the observation group.Meanwhile,the medical orders were pre-audited that the unreasonable ones were revised by informing the doctors.Combining the medical records,the pharmacists reviewed and commented the problem records screened out from the system,which were then improved with the administrative auxiliary management measures.The reasonableness of two groups were evaluated by six aspects:prophylactic medication,use intensity,improper combination of drugs,long course of treatment,unreasonable usage and dosage and unreasonable administration route.Results Through prescription pre-review and administrative management assistant measures,the phenomenon of irrational drug use was significantly reduced(from 78%to 19.5%),the situation of preventive drug use without high-risk factors was improved(from 81.92%to 31.98%),so that use intensity was significantly reduced.Conclusion Prescription preapproval intervention may make effort to promote the rational use of PPIs during the perioperative period.
