摘要
目的探讨CT灌注参数动脉增强分数(AEF)值在评估肝癌经导管动脉化疗栓塞疗法术后疗效中的作用。方法回顾性分析2016年4月—2018年4月本院收治的40例行肝癌经导管动脉化疗栓塞疗法术治疗患者的研究资料,术前、术后均行CT扫描,分析CT灌注参数动脉增强分数值变化,观察其对疗效的评价价值。结果有效组术后AEF值为(0.35±0.08),显著低于术前的(0.50±0.07),差异具有统计学意义(P<0.05);无效组术后AEF值(0.43±0.07)与术前(0.49±0.08)比较,差异无统计学意义(P>0.05)。术后有效组AEF值为(0.35±0.08),显著低于无效组的(0.49±0.08),差异具有统计学意义(P<0.05);有效组和无效组术后甲胎蛋白AFP值均显著低于术前,其中有效组为(22.25±6.25),显著低于无效组的(28.17±6.18),差异具有统计学意义(P<0.05)。结论针对行肝癌经导管动脉化疗栓塞疗法术治疗患者采用常规CT扫描可结合CT灌注参数动脉增强分数值作为有效反映患者血流动力学指标,利于实现对术后疗效的早期评价,且安全性高,值得推广应用。
Objective To explore the role of CT perfusion parameter arterial enhancement fraction(AEF)in evaluating the efficacy of transcatheter arterial chemoembolization for liver cancer.Methods The research data of forty patients those treated with transcatheter arterial chemoembolization for liver cancer treated in our hospital from April 2016 to April 2018 was retrospectively analyzed.CT scans were performed before and after surgery to analyze the change of CT perfusion parameters arterial enhancement fraction,and observed its evaluation value for curative effect.Results The AEF value(0.35±0.08)in the effective group was significantly lower than that before the operation(0.50±0.07)(P<0.05).The AEF value in the ineffective group(0.43±0.07)was similar to the preoperative value(0.49±0.08),without significant difference(P>0.05).The postoperative AEF value of the effective group(0.35±0.08)was significantly lower than that of the ineffective group(0.49±0.08),with significant difference(P<0.05).The postoperative alpha-fetoprotein(AFP)value of both the effective group and the ineffective group were significantly lower than that before operation.The effective group(22.25±6.25)was significantly lower than the ineffective group(28.17±6.18)(P>0.05).Conclusions For patients undergoing transcatheter arterial chemoembolization for liver cancer,conventional CT scan can be combined with CT perfusion parameter arterial enhancement score as an effective indicator of hemodynamics of the patient,which is conducive to the early evaluation of postoperative curative effect and is safe.Worthy of promotion and application.
