摘要
目的:探讨化瘀排石汤对体外冲击波碎石术患者的疗效,并分析其对肾功能及炎症因子的影响。方法:选取2018年1月—2019年12月在我院经体外冲击波碎石术治疗的上尿路结石患者80例,按数表法将其随机分为对照组(n=40,基础抗炎治疗)和研究组(n=40,化瘀排石汤+基础抗炎治疗)。比较两组临床疗效、肾功能指标、血清炎性因子、中医证候积分。比较两组治疗期间与排石相关的并发症差异。结果:研究组临床总有效率为87.50%(35/40),高于对照组的62.50%(25/40)(P<0.05);治疗后两组尿微量白蛋白(mALB)、β2微球蛋白(β2-MG)、视黄醇结合蛋白(RBP)、血肌酐(Scr)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平均降低,且研究组以上各指标均低于对照组(P<0.05);治疗后,研究组尿频、腰痛、少腹急满、血尿及淋漓不爽症状积分均低于对照组;对比两组与排石相关的并发症发生率,研究组低于对照组(χ2=5.541,P=0.019)。结论:对于经体外冲击波碎石术治疗的上尿路结石患者采用化瘀排石汤治疗,具有较好的临床疗效,并可调节肾功能指标mALB、β2-MG、RBP等表达水平,降低炎性反应,减少患者与排石相关的并发症的发生,安全性高,临床治疗可参考。
Objective To explore the therapeutic effect of huayu paishi decoction on patients with extracorporeal shock wave lithotripsy and analyze its effect on renal function and inflammatory factors.Methods 80 patients with upper urinary calculi treated by extracorporeal shock wave lithotripsy in our hospital from January 2018 to December 2019 were selected.They were randomly divided into control group(n=40,basic anti-inflammatory treatment)and study group(n=40,huayu paishi decoction+basic anti-inflammatory treatment)according to the number table method.The clinical efficacy,renal function index,serum inflammatory factors and traditional Chinese medicine syndrome score were compared between the two groups.The differences of complications related to stone removal were compared between the two groups.Results The total clinical effective rate of the study group was 87.50%(35/40),which was higher than 62.50%(25/40)of the control group(P<0.05).After treatment,the levels of urinary microalbumin(mALB),β2-microglobulin(β2-MG),retinol binding protein(RBP),serum creatinine(SCR),high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)in the two groups were all decreased,and the above indexes in the study group were lower than those in the control group(P<0.05).After treatment,the syndrome scores of frequent urination,low back pain,low abdomen and acute fullness,hematuria and dripping discomfort of the study group were all lower than those of the control group.The incidence of complications related to stone removal of the study group was lower than that of the control group(χ2=5.541,P=0.019).Conclusion Huayu paishi decoction can be used for the treatment of upper urinary calculi treated by extracorporeal shock wave lithotripsy,which has a better clinical effect.It can adjust the expression levels of renal function indexes,such as mALB,β2-MG and RBP.Also,it can reduce the inflammatory response and the complications related to stone removal.It is safe and can be used for reference in clinica
