摘要
目的:对比分析加速康复外科集束化管理和医护一体加速康复外科围术期管理在腹腔镜结直肠癌根治术围手术期的应用价值。方法:选取2019年1月—2020年9月我院结直肠癌患者80例,按随机数字表法分为集束组(n=40)与一体组(n=40),均行腹腔镜结直肠癌根治术,集束组采用加速康复外科集束化管理,一体组采用医护一体化加速康复外科围术期管理。对比两组围术期情况、并发症发生率及术后VAS评分,对比两组患者术前、术后WBC、CRP水平变化。结果:集束组术后首次肛门排气、排便时间、术后住院时间较一体组短,住院费用较一体组低(P<0.05);术后2h、6h、12h集束组VAS评分较一体组低(P<0.05);集束组并发症发生率5.00%与一体组7.50%相比无显著差异(P>0.05);术后1天、3天、5天集束组WBC、CRP水平较一体组低(P<0.05)。结论:加速康复外科集束化管理和医护一体加速康复外科围术期管理应用于腹腔镜结直肠癌根治术围手术期中,均可降低术后并发症的发生率,其中加速康复外科集束化管理能减轻患者术后疼痛、炎性反应,加快患者术后恢复,相比于医护一体化管理,集束化管理更能减少医生和护士的工作量。
Objective:To compare and analyze the application value of cluster management of accelerated rehabilitation surgery and perioperative management of accelerated rehabilitation surgery in laparoscopic colorectal cancer radical surgery.Methods:From January 2019 to September 2020,80 patients with colorectal cancer in our hospital were divided into a cluster group(n=40)and an integrated group(n=40)according to the random number table method.For radical cancer surgery,the cluster group adopts accelerated rehabilitation surgery cluster management,and the integrated group adopts medical and nursing integration to accelerate rehabilitation surgery perioperative management.The perioperative conditions,complication rates and postoperative VAS scores of the two groups were compared,and the changes of WBC and CRP levels before and after the operation were compared between the two groups.Results:The first postoperative anal exhaustion,bowel movement time,and postoperative hospital stay in the cluster group were shorter than those in the integrated group,and the hospitalization cost was lower than that in the integrated group(P<0.05);the VAS scores in the cluster group at 2 h,6 h,and 12 h after operation were lower than those in the integrated group(P<0.05);the complication rate of5.00%in the bundle group was not significantly different from 7.50%in the integrated group(P>0.05);the WBC and CRP levels in the bundled group at 1 day,3 days,and 5 days after operation were lower than those in the integrated group(P<0.05).Conclusion:Accelerated rehabilitation surgery cluster management and medical care integrated accelerated rehabilitation surgery perioperative management can be used to reduce the incidence of postoperative complications during perioperative laparoscopic colorectal cancer radical surgery.Among them,accelerated rehabilitation surgery cluster management can Alleviate postoperative pain and inflammatory reaction of the patient,and speed up the patient’s postoperative recovery.Compared with the integrated medical and nu
