摘要
目的:研究马来酸依那普利叶酸片(EMFA)治疗慢性心衰(CHF)的疗效及其对心功能、血清同型半胱氨酸(Hcy)及胶原代谢的影响。方法:选择我院的CHF患者186例,随机均分为依那普利组(93例,依那普利+常规治疗)和EMFA组(93例,EMFA+常规治疗),均治疗6个月。观察两组治疗前后心功能、炎性因子水平及血清Hcy、I型胶原(ICTP)水平、Lee心衰积分等的变化。结果:治疗后,与依那普利组比较,EMFA组LVEF[(42.85±4.33)%比(46.54±4.72)%]、6min步行距离(6MWD)[(209.19±21.58)m比(253.29±26.51)m]均显著增加,左室收缩末期内径(LVESd)[(50.59±5.13)mm比(42.43±4.38)mm]、左室舒张末期内径(LVEDd)[(61.40±6.22)mm比(57.04±5.88)mm]均显著缩小,血清白介素(IL)-8[(32.22±3.35)ng/L比(28.75±2.92)ng/L]、高敏C反应蛋白(hsCRP)[(5.74±0.59)mg/L比(3.81±0.40)mg/L]、Hcy[(17.75±1.83)μmol/L比(10.96±1.15)μmol/L]、基质金属蛋白酶9(MMP-9)[(3468.91±352.05)ng/m比(2053.75±211.59)ng/m]、ICTP[(2377.94±239.05)ng/ml比(1606.87±163.96)ng/m]水平及Lee心衰积分[(4.58±0.47)分比(2.85±0.31)分]显著降低(P均=0.001)。结论:马来酸依那普利叶酸可纠正CHF患者心肌胶原代谢失衡,抑制心室重塑,提高心功能。
Objective:To study therapeutic effect of enalapril maleate and folic acid tablets(EMFA)on chronic heart failure(CHF)and its impact on heart function,serum homocysteine(Hcy)level and collagen metabolism.Methods:A total of 186 CHF inpatients from our hospital were randomly and equally divided into enalapril group(received enalapril+routine therapy)and EMFA group(received EMFA+routine therapy),both groups were treated for six months.Heart function,levels of inflammatory factors,serum Hcy,type I collagen(ICTP)and Lee heart failure score were observed and compared between two groups before and after treatment.Results:Compared with enalapril group after treatment,there were significant rise in LVEF[(42.85±4.33)%vs.(46.54±4.72)%]and 6min walking distance(6MWD)[(209.19±21.58)m vs.(253.29±26.51)m],and significant reductions in left ventricular end-systolic dimension(LVESd)[(50.59±5.13)mm vs.(42.43±4.38)mm],left ventricular end-diastolic dimension(LVEDd)[(61.40±6.22)mm vs.(57.04±5.88)mm],serum levels of interleukin(IL)-8[(32.22±3.35)ng/L vs.(28.75±2.92)ng/L],high sensitive C reactive protein(hsCRP)[(5.74±0.59)mg/L vs.(3.81±0.40)mg/L],Hcy[(17.75±1.83)μmol/L vs.(10.96±1.15)μmol/L],matrix metalloproteinase(MMP)-9[(3468.91±352.05)ng/m vs.(2053.75±211.59)ng/m],ICTP[(2377.94±239.05)ng/ml vs.(1606.87±163.96)ng/m]and Lee heart failure score[(4.58±0.47)scores vs.(2.85±0.31)scores]in EMFA group,P=0.001 all.Conclusion:EMFA can correct myocardial collagen metabolism imbalance,improve cardiac function,inhibit ventricular remodeling in CHF patients.
