摘要
目的探讨医疗失效模式与效应在消毒供应室护理安全管理中的应用效果。方法本科室从2018年7月将医疗失效模式与效应应用于微创手术器械消毒灭菌过程,组建医疗失效模式与效应小组,制定流程图、评估可能失效环节、分析存在的原因和对应的解决方法,用于消毒供应室质量安全管理中。比较医疗失效模式实施前后6个月科室工作人员在微创手术器械平均回收发放时间,清洗消毒、检查包装、灭菌的合格率及器械损坏率。结果实施后,消毒供应室微创手术器械平均回收时间由实施前的5.68 min缩短至3.12 min(P<0.05),平均发放时间由实施前的10.33 min缩短至6.59 min(P<0.05);实施后,微创手术器械清洗消毒、检查包装、灭菌合格率分别为99.40%、99.80%、99.80%,均明显高于实施前的95.00%、96.40%、95.40%(P<0.05),器械破损率也由实施前的7.00%降低至2.00%(P<0.05);科室工作人员的工作质量、服务态度、风险防范意识和安全识别能力均较实施前明显提高(P<0.05)。结论引进医疗失效模式后能明显提高消毒供应室工作人员的工作效率,提高科室微创手术器械质量管理,增强科室人员的服务意识。
Objective To explore the application effects of medical failure modes and effects in the nursing safety management of disinfection supply rooms.Methods The undergraduate room has applied medical failure modes and effects to the disinfection and sterilization process of minimally invasive surgical instruments from July 2018,setting up medical failure mode and effect team,formulate flowcharts,evaluate possible failure links,analyzed the causes and corresponding.The solution was used in the quality and safety management of the disinfection supply room.Compared the average recovery and release time of minimally invasive surgical instruments by department staff in the 6 months before and after the implementation of the medical failure mode,the pass rate of cleaning and disinfection,packaging inspection,and sterilization,and the device damage rate.Results After implementation,the average recovery time of minimally invasive surgical instruments in the disinfection supply room was shortened from the original 5.68 min to 3.12 min(P<0.05),and the average distribution time was shortened from the original 10.33 min to 6.59 min(P<0.05);after the implementation,the pass rates of minimally invasive surgical instruments in cleaning,disinfection,packaging inspection,and sterilization were 99.40%,99.80%and 99.80%,which were significantly higher than the 95.00%,96.40%and 95.40%before implementation(P<0.05),the damage rate of the device was also reduced from 7.00%before implementation to 2.00%(P<0.05);the work quality,service attitude,risk prevention awareness and safety identification ability of the department staff were also significantly improved compared with that before the implementation(P<0.05).Conclusion The introduction of medical failure modes can significantly improve the work efficiency of the staff in the disinfection supply room,improve the quality management of minimally invasive surgical instruments in the department,and enhance the service awareness of the staff in the department.
