摘要
目的探讨白蛋白与碱性磷酸酶比值(albumin-to-alkaline phosphatase ratio,AAPR)与乳腺癌新辅助化疗疗效的相关性。方法将2014年1月至2020月1月在海口市人民医院接受新辅助化疗的120例乳腺癌患者纳入化疗组,将该院同期120例健康体检女性纳入健康组。根据AAPR截断值将乳腺癌患者分为高AAPR组(70例)和低AAPR组(50例)。比较化疗前不同AAPR与乳腺癌患者临床特征、化疗后疗效的关系,采用多因素Logistic回归分析探讨影响乳腺癌患者新辅助化疗疗效的相关因素。结果化疗组患者AAPR显著低于健康组(P<0.05),AAPR诊断乳腺癌患者的曲线下面积为0.721,截断值为0.54,灵敏度为79.52%,特异度为89.47%;高AAPR组患者人表皮生长因子受体-2(human epider mal growth factor receptor type-2,HER-2)阳性表达、Ki-67阳性表达比例均显著低于低AAPR组(均P<0.05);高AAPR组患者缓解率和病理完全缓解率均显著高于低AAPR组(均P<0.05);多因素Logistic回归分析结果显示,AAPR≤0.54、Ki-67阳性、HER-2阳性均是乳腺癌患者新辅助化疗疗效的影响因素(均P<0.05)。结论AAPR可作为预测乳腺癌新辅助化疗疗效的有效指标,且化疗前AAPR>0.54的乳腺癌患者新辅助化疗疗效更佳。
Objective To investigate the relationship between albumin-to-alkaline phosphatase ratio(AAPR)and efficacy of neoadjuvant chemotherapy for breast cancer.Method One hundred and twenty patients with breast cancer who received neoadjuvant chemotherapy in Haikou People's Hospital from January 2014 to January 2020 were selected as chemotherapy group,and 120 healthy women with physical examination in the same period in that hospital were selected as healthy group.According to the AAPR cutoff value,breast cancer patients were divided into high AAPR group(70 cases)and low AAPR group(50 cases).The relationship between different AAPR values before chemotherapy and the clinical characteristics of breast cancer patients and the curative effects after chemotherapy were compared.Logistic regression analysis was used to analyze the related factors affecting the efficacy of neoadjuvant chemotherapy for breast cancer.Result The AAPR of chemotherapy group was significantly lower than that of healthy group(P<0.05).The area under the curve of AAPR in the diagnosis of breast cancer patients was 0.721,the cut-off value was 0.54,the sensitivity was 79.52%,and the specificity was 89.47%.The proportions of human epidermal growth factor receptor type-2(HER-2)positive expression and Ki-67 positive expression in high AAPR group were lower than those in low AAPR group(all P<0.05).The response rate and pathologic complete response rate in high AAPR group were significantly higher than those in low AAPR group(all P<0.05).Logistic regression analysis results showed that AAPR≤0.54,Ki-67 positive,HER-2 positive were the influencing factors of neoadjuvant chemotherapy for breast cancer(all P<0.05).Conclusion AAPR can be used as an effective factor to predict the efficacy of neoadjuvant chemotherapy for breast cancer,and the curative effect of neoadjuvant chemotherapy is better in patients with breast cancer with AAPR>0.54 before neoadjuvant chemotherapy.
