N端前脑钠肽及红细胞分布宽度对先天性心脏病孕妇分娩小于孕龄儿的预测价值 被引量:2

Predictive Value of NT-proBNP,RDW for Delivery Small for Gestational Age in Women with Congenital Heart Disease

摘要 目的:探讨N端前脑钠肽(NT-proBNP)、红细胞分布宽度(RDW)对先天性心脏病(CHD)孕妇分娩小于孕龄儿(SGA)的预测价值。方法:收集2018年12月至2020年3月于北部战区总医院进行妊娠合并心脏病多学科会诊并于该院分娩的84例CHD孕妇的临床资料,根据SGA的诊断标准将CHD孕妇分为非SGA组(72例)和SGA组(12例),并分析NT-proBNP、RDW、血氧饱和度(SaO2)、超敏C反应蛋白(hs-CRP)对预测SGA的诊断效能。结果:单因素分析显示两组间NTproBNP、RDW及SaO2的差异有统计学意义(P<0.05),hs-CRP的差异无统计学意义(P>0.05)。多因素Logistic分析显示NT-proBNP及RDW是CHD孕妇分娩SGA的独立预测因子(P<0.05)。受试者工作曲线(ROC)分析显示NT-proBNP的曲线下面积(AUC)为0.951,敏感度89.5%,特异度96.9%,阈值为91.00 pg/ml;RDW的AUC为0.697,敏感度100.0%,特异度31.3%,阈值为19.00%。Pearson相关性分析显示NT-proBNP及RDW与新生儿出生体质量呈负相关关系(r=-0.538,-0.354;P<0.05)。结论:可以采用NT-proBNP与RDW评估CHD孕妇分娩SGA的发生风险。NT-proBNP及RDW与CHD孕妇新生儿出生体质量有一定相关性,可协同监测胎儿生长发育情况。 Objective:To explore the predictive value of maternal risk factors in pregnant women with congenital heart disease(CHD)who deliver small for gestational age(SGA).Methods:The data of 84 pregnant women with CHD from December 2018 to March 2020 in the North War area general hospital were retrospectively analyzed.The offspring of CHD pregnant women were divided into non SGA group(72 cases)and SGA group(12 cases).The diagnostic efficacy of NT-proBNP,RDW,SaO2,and hs-CRP in predicting SGA were ananlyzed.Results:Univariate analysis showed that maternal NT-proBNP,RDW and SaO2 were significantly different between the two groups(P<0.05),while hs-CRP was not significantly different(P>0.05).Multivariate logistic regression analysis showed that NT-probNP and RDW were independent predictors of SGA in pregnant women with CHD(P<0.05).The subject working characteristic curve analysis showed that the predictive ability of NT-proBNP was 0.951,with the sensitivity of 89.5%and the specificity of 96.9%.The predictive ability of RDW was 0.697,with the sensitivity of 100.0%and the specificity of 31.3%.Pearson correlation analysis shows that NT-proBNP and RDW are negatively correlated with newborn birth weight(r=-0.538,-0.354;P<0.05).Conclusions:NT-proBNP and RDW can be used to evaluate the risk of SGA in pregnant women with CHD.NT-proBNP and RDW have a certain correlation with the birth weight of the offspring of pregnant women with CHD,which can be used to mointor the growth of the fetus.
