摘要
目的分析加速康复外科理念在甲状腺围手术期患者中的应用价值。方法选取新疆阿克苏地区第一人民医院耳鼻咽喉科2017年1月至2018年12月收治的甲状腺疾病行甲状腺手术的患者100例,并采用随机数字表法分为观察组和常规组,每组50例。常规组实施常规护理,观察组在常规护理基础上实施基于加速康复外科理念的护理,比较两组的住院指标、并发症情况,并对比两组围术期的自我护理能力。结果观察组术后首次下床时间(2.45±0.62)d,住院时间(5.04±0.78)d,短于常规组的(4.13±0.69)d、(8.02±0.76)d,观察组住院治疗费用为(0.52±0.10)万元,少于常规组的(0.73±0.13)万元,差异具有统计学意义(P<0.05)。观察组并发症总发生率较常规组低,差异有统计学意义(P<0.05)。两组术前自我护理能力比较,差异无统计学意义(P>0.05);观察组术后第1d、3d、5d的自我护理能力评分均较常规组高,差异有统计学意义(P<0.05)。结论甲状腺手术患者接受基于加速康复外科理念的干预,可促进康复进程的有效加快,减少并发症,利于缩短住院时间,减少医疗支出。
Objective To analyze the application value of the concept of accelerated rehabilitation surgery in patients during thyroid perioperative period.Methods A total of 100 patients with thyroid disease who underwent thyroid surgery admitted to the Department of Otolaryngology in our hospital from January 2017 to December 2018 were selected and divided into observation group and routine group by random number table,with 50 cases in each group.The routine group were performed routine nursing care,and the observation group were treated with nursing based on the concept of accelerated rehabilitation surgery on the basis of routine care.The hospitalization indicators and complications between two groups were compared,and the perioperative self-care ability between two groups was compared.Results In the observation group,leaving bed time after operation was(2.45±0.62)days and the hospital stay was(5.04±0.78)days,and they were shorter than those in the routine group which were(4.13±0.69)days and(8.02±0.76)days.The treatment cost of the observation group was(0.52±0.10)ten thousand Yuan,which was less than that of conventional group which was(0.73±0.13)ten thousand Yuan,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the routine group(P<0.05).Preoperative self-care ability between two groups was not statistically significant(P>0.05).The self-care ability scores of the observation group on the 1st,3rd and 5th day after operation were higher than those of the conventional group(P<0.05).Conclusions The nursing care method for patients undergoing thyroid surgery based on the concept of accelerated rehabilitation surgery can effectively accelerate the rehabilitation process,reduce complications,shorten the length of hospital stay,and reduce medical expenditures.
