摘要
目的:探讨冠状动脉CT造影血液循环速度与增强效果的关系、对图像质量的影响及解决方案。方法:用小剂量团注法测量主动脉根部对比剂达峰值时间,根据循环时间分为循环较快组(A组)、中等组(B组)、缓慢组(C组)。C组随机分为C1、C2、C3组,C1组对比剂碘浓度、注射速率与A组和B组相同,均使用300 mgI/mL对比剂,以5 mL/s注射速率注射;C2组使用350 mgI/mL对比剂,但以5.5 mL/s注射速率注射;C3组使用370 mgI/mL对比剂,以5.5 mL/s注射速率注射;统计分析各组间增强效果差异及血液循环速度与增强效果的关系。结果:单因素方差分析(ANOVA)显示各组间增强效果总体有差异,Tukey HSD法全面两两比较显示各组间增强效果有差异,不同注射方案间增强效果、图像质量有差异;行Pearson相关分析显示血液循环速度与增强效果呈高度负相关。结论:血液循环速度是影响冠状动脉CT造影增强效果的重要因素,在设计对比剂注射方案时,对血液循环明显缓慢者,要增加注射速率和增加对比剂碘浓度,以保证冠状动脉检查的质量。
Objective:To explore the relationship between the blood circulation speed and enhancement effect of coronary artery CT angiography and its influence on image quality and the solution.Methods:Peak contrast time of aortic root was measured by small dose bolus,and according to the circulation time,it was divided into fast circulation group(group A),medium circulation group(group B)and slow circulation group(group C).Group C was randomly divided into groups C1,C2 and C3.The iodine concentration of the contrast agent and injection rate in group C1 were the same as those in group A and group B,using the 300 mgI/mL of contrast agent,the injection rate was 5 mL/s.C2 group was injected with the contrast agent of 350 mgI/mL at an injection rate of 5.5 mL/s.C3 group was injected with the contrast agent of 370 mgI/mL at an injection rate of 5.5 mL/s.Enhancement effect between the groups and the relationship between blood circulation speed and enhancement effect were analyzed.Results:One-way analysis of variance(ANOVA)showed that there were overall differences in enhancement effects between groups.The Tukey HSD comprehensive pairwise comparison showed that the enhancement effect was different among the groups.The enhancement effect and image quality were different among different injection schemes.Pearson correlation analysis showed that the blood circulation velocity was highly negatively correlated with the enhancement effect.Conclusion:Blood circulation velocity is an important factor affecting the enhancement of coronary angiography.When designing the contrast agent injection program,the injection rate and the contrast iodine concentration should be increased to ensure the quality of coronary examination for the patients with significantly slow blood circulation.
