摘要
目的:系统评价安宫牛黄丸联合西药治疗肝性脑病(Hepatic Encephalopathy,HE)的疗效以及安全性,为综合治疗肝性脑病提供参考。方法:计算机检索Pubmed、Embase、The Cochrane Library、中国知识资源总库(China National Knowledge Infrastructure,CNKI)、中国生物医学文献数据库(China Biology Medicine disc,CBM)、万方数据库(WanFang Data)、维普数据库(VIP)公开发表的文献,纳入随机对照试验。由两位评价者独立对纳入研究进行筛选和资料提取并交叉核对。采用Review Manager 5.3软件进行分析评价。结果:共纳入9项研究,包括640例患者(其中实验组321例,对照组319例)。Meta分析结果显示:治疗组总有效率、谷丙转氨酶(Alanine Transaminase,ALT)的变化、血氨的变化、总胆红素(Total Bilirubin,TBIL)的变化均显著优于对照组,差异具有统计学意义。结论:安宫牛黄丸联合西药治疗肝性脑病能显著提高总有效率,降低谷丙转氨酶、血氨及总胆红素,且安全性等各方面较单纯西药治疗更好,拓宽了肝性脑病的治法思路,值得在临床参考和推广。
Objective:To evaluate the efficacy and safety of An’gong Niuhuang Wan(安宫牛黄丸)with western medicine on hepatic encephalopathy,and to provide references for comprehensive treatment of hepatic encephalopathy.Methods:The published literatures were searched in Pubmed,Embase,The Cochrane Library,CNKI,CBM,WanFang Data,VIP databases and included in randomized controlled trials.The literatures included in the study were screened,relevant information was extracted,cross check was executed,and all the above operations were performed independently by two evaluators.Results:9 studies were included,including 640 cases(321 cases in the experimental group and 319 cases in the control group).The total efficiency,the change of alanine aminotransferase,blood ammonia and total bilirubin in the treatment group were significantly better than the control group,with statistically significant differences.Conclusion:An’gong Niuhuang Wan with western medicine on hepatic encephalopathy can significantly improve the total efficiency,reduce alanine aminotransferase,blood ammonia and total bilirubin,which broadens the treatment thoughts on hepatic encephalopathy,and is worthy of clinical reference and promotion.
