摘要
目的探讨髓过氧化物酶(MPO)与氧化低密度脂蛋白(OX-LDL)对缺血性胸痛患者的临床诊断价值。方法对2017年10月一2019年6月在苏州九龙医院急诊科就诊的怀疑缺血性胸痛患者进行前瞻性研究,按照胸痛中心急性胸痛诊治流程,所有怀疑缺血性胸痛患者入抢救室进行诊治,在患者签署知情同意书后,即刻,外周静脉采血获取血液样本,采用酶联免疫吸附试验法测定血浆MPO和OX-LDL水平,并记录直接经皮冠状动脉介入术(PPCI)检查结果。根据检查结果分为缺血性胸痛组和非缺血性胸痛组,采用统计学软件SPSS 23.0对两组患者血浆MPO和OX-LDL水平进行比较,并分析所有入选患者两种血浆学指标间的相关性。结果入选患者共计142例,缺血性胸痛组(99例),非缺血性胸痛组(43例),两组患者血浆MPO水平分别为94.54(77.65,108.85)ug/L和84.48(79.32,94.58)pg/L,组间比较差异有统计学意义(P=0.012);两组患者血浆OX-LDL水平分别为14.02(10.32,18.25)ug/L和10.87(8.52,12.87)ug/L,组间比较差异有统计学意义(P=0.009)。同时发现所有入选患者血浆髓过氧化物酶与氧化低密度脂蛋白水平呈正相关(=0.845,P<0.01)。结论联合检测血浆髓过氧化物酶与氧化低密度脂蛋白对缺血性胸痛患者的临床诊断有一定的参考价值。
Objective To investigate the clinical diagnostic value of myeloperoxidase(MPO)and oxidized low-densitylipoprotein(OX-LDL)in patients with ischemic chest pain.Methods A prospective study was conducted on patientswith suspected ischemic chest pain who visited the emergency department of Suzhou Kowloon Hospital from October2017 to June 2019.According to the diagnosis and treatment procedure of acute chest pain in chest pain center,all pa-tients suspected of ischemic chest pain were admitted to the emergency room for diagnosis and treatment.Immediatelyafter the patient signed the informed consent,peripheral vein blood was collected to obtain blood samples.Plasma MPOand OX-LDL levels were measured by Enzyme-linked immune sorbent assay(ELISA)and results of Primary Percuta-neous Coronary Intervention(PPCI)were recorded.According to the examination results,patients were divided into theischemic chest pain group and the non-ischemic chest pain group.Statistical software SPSS 23.0 was used to comparethe plasma MPO and OX-LDL levels between the two groups of patients,and to analyze the correlation between the twoplasma indicators in all enrolled patients.Results Among the 142 enrolled patients,99 patients were in ischemic chestpain group and 43 patients were in non-ischemic chest pain group.The plasma MPO levels of patients in the twogroups were 94.54(77.65,108.85)ug/L and 84.48(79.32,94.58)pg/L,the difference between two groups was statisti-cally significant(P=0.012).The plasma OX-LDL levels in the two groups were 14.02(10.32,18.25)ug/L and 10.87(8.52,12.87)ug/L,the difference between two groups was statistically significant(P=0.009).Meanwhile the levels ofplasma MPO and OX-LDL were positively correlated(r=0.845,P<0.01)in all enrolled patients.Conclusion The com-bined detection of plasma MPO and OX-LDL is valuable for clinical diagnosis of ischemic chest pain.
