摘要
目的:探讨利培酮与奥氮平对女性精神分裂症患者脂肪肝和血脂代谢指标的影响。方法:选取2015年1月至2017年12月在荆州市精神卫生中心就诊的1000例女性精神分裂症患者为研究对象,应用随机数字表法将患者分为A组(口服6 mg/d利培酮治疗)和B组(口服15 mg/d奥氮平治疗)。同期选取荆州市精神卫生中心500例健康体检者作为对照组,比较3组受试者脂肪肝检出率、总胆固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白变化情况。结果:B组与A组受试者脂肪肝检出率均高于对照组(23.35%vs.18.07%vs.6.66%,P<0.05)。12个月后,对照组受试者总胆固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白变化不明显,差异无统计学意义(P>0.05),A组和B组患者总胆固醇、甘油三酯、低密度脂蛋白均高于基线时(P<0.001),B组升高更为明显,差异具有统计学意义(P<0.05)。结论:接受奥氮平治疗的女性精神分裂症患者脂肪肝和血脂代谢异常检出率较利培酮组高。在精神分裂症的治疗中还需关注药物副作用。
Objective:To explore the effect of risperidone and olanzapine on fatty liver and lipid metabolism in female schizophrenics.Methods:One thousand female patients with schizophrenia who met the diagnostic criteria of the international classification of diseases(edition 10)in Jingzhou mental health center from January 2015 to December 2017 were selected as the research objects.Patients were divided into A group(treated with 6mg/d risperidone)and B group(treated with 15mg/d olanzapine)using a random number table.During the same period,500 healthy subjects in Jingzhou mental health center were selected as the control group,the changes of fatty liver,total cholesterol,triglyceride,low-density lipoprotein,and high-density lipoprotein in the three groups were compared.Results:The detection rate of fatty liver in group B and group A was higher than that in the control group(23.35%vs.18.07%vs.6.66%,P<0.05).After 12 months,there were no significant changes in total cholesterol,triglyceride,low-density lipoprotein,and high-density lipoprotein in the control group,and the differences were not statistically significant(P>0.05).When the total cholesterol,triglyceride and low-density lipoprotein in group A and group B were all higher than the baseline,the increase was more obvious in group B,and the difference was statistically significant(P<0.05).Conclusion:The detection rates of fatty liver and abnormal lipid metabolism in female schizophrenia patients treated with olanzapine were higher than those in the risperidone group.In the treatment of schizophrenia,we need to pay attention to the side effects of drugs.
