颅脑超声评估新生儿脑发育与孕周及出生体质量的相关性 被引量:1

The correlation between neonatal brain development evaluated by craniocerebral ultrasound and gestational age and birth weight

摘要 目的通过颅脑超声评估新生儿脑发育与孕周、出生体质量的相关性,分析其在新生儿脑发育中的临床应用价值。方法收集本院2017年6月至2019年12月收治的120例新生儿的临床资料,根据出生孕周分为A组20例(29~32周)、B组28例(>32~34周)、C组32例(>34~36周)、D组40例(>36周)。4组于生后3 d内通过超声测量不同区域脑组织平均灰度值、胼胝体和小脑蚓矢状长度,分析其相关性。结果各组新生儿出生时不同区域脑组织平均灰度值从小到大分别为D组、C组、B组、A组,胼胝体和小脑蚓平均矢状长度从小到大分别为A组、B组、C组、D组(均P<0.05);相关性研究显示,孕周与基底节、额叶、顶叶及枕叶白质平均灰度值均呈负相关(r=-0.673、-0.594、-0.557、-0.521,均P<0.05),出生体质量与基底节、额叶、顶叶及枕叶白质平均灰度值均呈负相关(r=-0.598、-0.543、-0.526、-0.497,均P<0.05)。胼胝体矢状长度与孕周、出生体质量均呈正相关(r=0.466、0.538,均P<0.05),小脑蚓矢状长度与孕周、出生体质量均呈正相关(r=0.435、0.484,均P<0.05)。结论颅脑超声检查费用低、操作方便且可重复性强,可有效评估新生儿脑发育情况,为临床干预提供可靠影像学依据。 Objective To evaluate the correlation between neonatal brain development evaluated by craniocerebral ultrasound and gestational age and birth weight,and to analyze its clinical application value in neonatal brain development.Methods Clinical data of 120 newborns in our hospital from June 2017 to December 2019 were collected and were divided into group A(20 cases,29-32 weeks),group B(28 cases,32-34 weeks),group C(32 cases,34-36 weeks),and group D(40 cases,>36 weeks).The average gray value of brain tissues,sagittal length of callosum and vermis were measured by ultrasound in 4 groups within 3 days after birth,and the correlation was analyzed.Results At the time of birth,the average gray value of brain tissues in different regions of each group was from small to large in group D,group C,group B,and group A,the sagittal length of callosum and vermis were from small to large in group A,group B,group C,and group D(all P<0.05).Correlation studies showed that gestational age was negatively correlated with the average gray values of basal ganglia,frontal lobe,parietal lobe,and occipital lobe white matter(r=-0.673,-0.594,-0.557,-0.521,all P<0.05),and birth weight was negatively correlated with the average gray values of basal ganglia, frontal lobe, parietal lobe, and occipital lobe white matter (r =-0.598, -0.543, -0.526,-0.497, all P<0.05). The sagittal length of callosum was positively correlated with gestational age andbirth weight (r=0.466, 0.538, both P<0.05), and the sagittal length of vermis was positivelycorrelated with gestational age and birth weight (r=0.435, 0.484, both P<0.05). ConclusionCraniocerebral ultrasonography has the advantages of low cost, easy operation, and highrepeatability, which can effectively evaluate the development of neonatal brain and provide reliableimaging basis for clinical intervention.
