摘要
目的:对集束化护理改善脑卒中病人吞咽障碍的效果进行系统评价。方法:利用计算机检索维普、万方数据库(WanFang Data)、中国期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)、PubMed、the Cochrane Library数据库有关脑卒中后吞咽障碍病人实施集束化护理的随机对照试验研究,利用RevMan 5.3统计软件进行Meta分析。结果:最终纳入11篇文献,合计846例病人。Meta分析结果显示,试验组在改善病人吞咽功能和促进神经功能恢复方面优于对照组,吞咽功能评分合并效应[OR=0.26,95%CI(0.20,0.36),P<0.00001],NIHSS评分合并效应[MD=-4.78,95%CI(-5.62,-3.94),P<0.00001];慢性病生存质量和日常生活活动能力评分高于对照组,因合并后存在异质性,只做定性描述。结论:现有证据表明,对脑卒中后并发吞咽障碍病人实施集束化护理可促进吞咽和神经功能的恢复,提高生活质量和日常生活能力。然而鉴于本次分析涵盖的研究有限,且总体质量偏低,期望增加更高质量、更大样本的研究以进一步验证其有效性。
Objective:To systematically evaluate effect of cluster nursing on improving dysphagia after stroke.Methods:Randomized controlled trial on cluster nursing for patients with dysphagia after stroke were searched form VIP,WanFang Data,CNKI,CBM,PubMed and the Cochrane Library.RevMan 5.3 software was used for Meta-analysis.Results:A total of 11 literatures were included,involving 846 cases.Meta-analysis results showed that the experimental group was better than the control group in improving the swallowing function and promoting the recovery of neurological function,swallowing function[OR=0.26,95%CI(0.20,0.36),P<0.00001].NIHSS score[MD=-4.78,95%CI(-5.62,-3.94),P<0.00001].The quality of life and activities of daily living(ADL)scores of chronic diseases were higher than those of the control group.However,due to the heterogeneity after merger,only qualitative description was made.Conclusions:Current evidences showed that cluster nursing could promote the recovery of swallowing and nerve function,improve the quality of life and the ability of daily life.However,in view of the limited research included in this analysis,and the low overall quality,large samples and high-quality trials are expected to be added in the later stage to further verify its effectiveness.
