摘要
目的分析初次髋关节置换术后行髋关节翻修的原因,发现引起髋关节翻修的危险因素,为临床上降低髋关节置换术后的翻修率提供方法指导。方法回顾性分析2012年8月至2019年10月行髋关节翻修术的85例患者,设定纳入与排除标准,其中20例符合标准的患者,作为研究组。选择同期在骨科行初次髋关节置换手术未行髋关节翻修术的40例患者作为对照组。结果单因素分析显示,两组在BMI、文化程度、初次髋关节置换原因、初次髋关节置换材料等方面比较差异有统计学意义(P<0.1)。多因素二元Logistic回归分析显示BMI(OR=1.897,P=0.011)、文化程度(OR=0.048,P=0.006)、股骨颈骨折(OR=12.538,P=0.044)、初次关节置换材料(OR=7.889,P=0.036)是髋关节翻修的影响因素。结论肥胖、股骨颈骨折、初次髋关节置换的假体材料为骨水泥是髋关节置换术后翻修的危险因素,文化程度高是其保护因素,临床工作中应给予特别关注。
Objective To analyze the causes of hip revision after first hip arthroplasty and find out the risk factors of hip revision,so as to provide methods and guidance for reducing revision rate after hip arthroplasty.Methods 85 patients who underwent hip revision surgery in our hospital from August 2012 to October 2019 were retrospectively analyzed.Inclusion and exclusion criteria were set,among them,20 patients who met the criteria were served as study group.40 patients who underwent primary hip artificial arthroplasty in our hospital and did not undergo hip revision were selected as control group.Results Univariate analysis showed that the two groups had statistically significant differences in BMI,education levels,reasons and materials for the first hip arthroplasty(P<0.1).Multivariate logistic regression analysis showed that BMI(OR=1.897,P=0.011),education level(OR=0.048,P=0.006),femoral neck fracture(OR=12.538,P=0.044),first hip arthroplasty material(OR=7.889,P=0.036)were the influencing factors of hip revision.Conclusion Obesity,femoral neck fracture and the prosthesis material of first hip arthroplasty is bone cement are the risk factors of revision after hip arthroplasty,and high education level is protective factor,special attention should be paid in clinical work.
