摘要
目的观察帕金森病(PD)伴焦虑患者大脑半球间体素镜像同伦连接性(VMHC),并探索PD伴发焦虑的神经病理机制。方法收集112例PD患者,以及54例健康对照者(HC)的rs-fMRI数据,根据汉密尔顿焦虑量表(HAMA)将PD组分为PD伴焦虑患者(PD-A)50例和PD不伴焦虑患者(PD-NA)62例。通过协方差分析(ANCOVA)和Post-Hoc t检验进行VMHC值的组间统计学比较,并用受试者工作特征(ROC)曲线评价差异脑区VMHC值对于帕金森伴焦虑的诊断效能。结果与PD-NA相比,PD-A的双侧额下回(IFG)三角部及顶盖部VMHC值减低,并且额下回VMHC值对于帕金森病伴焦虑的诊断效能较高,ROC曲线下面积为0.815。结论PD-A患者存在异常的脑功能活动,双侧IFG存在异常VMHC改变,提示IFG相关网络可能参与了焦虑的病理发生过程,可能是其潜在的影像生物学标志物。
Objective To investigate the alterations of functional connectivity between inter-hemisphere of Parkinson’s disease(PD)patients with anxiety using voxel-mirrored homotopic connectivity(VMHC),and to explore its pathological mechanism.Methods 112 PD patients and 54 healthy controls underwent resting state functional MRI(rs-fMRI).According to the Hamilton Anxiety Scale,112 PD patients were divided into 50 PD with anxiety(PD-A)and 62 PD without anxiety(PD-NA).ANCOVA and Post-Hoc t test were performed to assess intergroup difference of VMHC value.A receiver operating characteristic(ROC)curve was used to evaluate its diagnostic value.Results Compared to the PD-NA group,PD-A group showed decrease of VMHC value in the triangular and opercular parts of bilateral inferior frontal gyri(IFG)with high diagnostic efficiency and area under the ROC curve of 0.815.Conclusion This study suggests that anxiety in PD is associated with abnormalities in IFG-related network,and may be a potential imaging diagnostic biomarker.
