摘要
目的:观察温经汤加减方治疗寒凝血瘀型原发性痛经的临床疗效,探讨其治疗原发性痛经的作用机理,为临床应用及新药开发提供相关依据。方法:将确诊为寒凝血瘀型原发性痛经的门诊患者60例随机分为治疗组(30例,予以温经汤加减方)、对照组(30例,口服痛经宝颗粒)。治疗3个月经周期为一个疗程。观察两组患者治疗前后疼痛视觉模拟量表(Visual Analogue Scale,VAS)评分及中医证候积分改变,血液流变学指标的变化。结果:治疗组和对照组治疗后疼痛强度评分和中医证候评分均低于治疗前(P<0.05),说明两组均能改善疼痛,且治疗组的效果明显优于对照组;两组患者血液流变学相关指标全血粘度(高切、中切、低切)、血浆粘度、红细胞压积与正常范围相比有显著的差异(P<0.05),且治疗组在改善血液流变性方面明显优于对照组;治疗组总有效率高于对照组,差异具有统计学意义(P<0.05),治疗组在综合疗效方面优于对照组。结论:温经汤加减方对于寒凝血瘀型原发性痛经,能明显降低VAS评分和中医证候积分,有效改善痛经患者的临床症状,提高临床疗效;且能增加患者血液流动性,促进血液循环,改善子宫局部缺血缺氧状态。
Objective:To observe the clinical efficacy of the Wenjing decoction(温经汤)on primary dysmenorrhea of the Hanning Xueyu type(寒凝血瘀型),in order to explore its treatment mechanism to provide relevant evidence for clinical application and new medicine development.Methods:60 cases were randomly divided into the treatment group(30 cases,took the Wenjing decoction)and the control group[30 cases,received the Tongjingbao granules(痛经宝颗粒)].1 course of treatment lasted for 3 menstrual cycles.Results:Pain intensity scores and TCM syndrome scores in the groups were lower than before treatment(P<0.05),and those in the treatment group was significantly better than the control group in reducing pain and improving TCM syndromes.There were significant differences in hemorheology-related indicators,such as whole blood viscosity,plasma viscosity and hematocrit,with the reference range of normal women’s hemorheology(P<0.05).The effect in the treatment group is significantly better than the control group in improving blood rheology.The total efficiency in the treatment group was higher than the control group(P<0.05).The terms of comprehensive efficacy in the treatment group was superior to the control group.Conclusion:The Wenjing decoction on primary dysmenorrhea of the Hanning Xueyu type can significantly reduce the VAS score and TCM syndrome score,effectively ameliorate the clinical symptoms,improve clinical efficacy,improve blood flow,promote blood circulation,and ameliorate uterine ischemia and hypoxia state.
