痛风性关节炎患者UA水平与炎性指标的相关性 被引量:3

Correlation between UA levels and inflammatory indexes in patients with gouty arthritis

摘要 目的探讨痛风性关节炎(GA)患者血尿酸(UA)水平与炎性指标的相关性。方法随机选取云南省第一人民医院2017年1月至2018年2月收治的GA患者30例(GA组),高尿酸血症(HuA)患者34例(HuA组),同时随机选取健康体检者40例(对照组),进行前瞻性研究。检测并比较3组研究对象UA,红细胞沉降率(ESR),C-反应蛋白(CRP),白介素(IL)-2、IL-4、IL-6、IL-8、IL-10、IL-1β、IL-12p70,干扰素-γ(INF-γ),肿瘤坏死因子-α(TNF-α)水平;分析3组研究对象UA水平与炎性指标之间的相关性。结果GA组和HuA组患者血清UA、ESR、CRP、INF-γ、TNF-α、IL-6、IL-2、IL-12p70、IL-1β、IL-8水平均显著高于对照组,除UA外,GA组上述指标均显著高于HuA组;GA组和HuA组患者血清IL-10水平和GA组患者血清IL-4水平均显著低于对照组,GA组患者血清IL-10水平显著低于HuA组(均P<0.05);Spearman相关分析结果,HuA患者UA水平与ESR、TNF-α、IL-2水平呈显著正相关性(r=0.449、0.452、0.398),与IL-10、IL-1β水平呈显著负相关(r=-0.359、-0.387);对照组患者UA水平与TNF-α水平呈显著负相关,与IL-1β水平呈显著正相关(r=-0.339、0.387)(均P<0.05)。结论高UA水平会引起促炎细胞因子及抑炎因子的变化,GA急性发作期,UA水平的升高既不会影响炎症指标,也不会引起炎性细胞因子水平的变化。 Objective To investigate the correlation between blood uric acid(UA)levels and inflammatory indexes in patients with gouty arthritis(GA).Methods A total of 30 GA patients(GA group)and 34 patients with hyperuricemia(HuA)(HuA group)who were admitted to The First People's Hospital of Yunnan Province from January 2017 to February 2018 were selected randomly,at the same time,40 healthy patients were selected randomly(Control group)to conduct a prospective study.UA,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),interleukin(IL)-2,IL-4,IL-6,IL-8,IL-10,IL-1β,IL-12p70,interferon-γ(INF-γ),tumor necrosis factor-α(TNF-α)levels of the 3 groups were detected and compared;the correlation between UA and inflammatory indicators of the 3 groups were analysed.Results The levels of UA,ESR,CRP,INF-γ,TNF-α,IL-6,IL-2,IL-12p70,IL-1β,and IL-8 of patients in the GA and HuA groups were significantly higher than those in the control group;excepted UA,the above indexes of patients in the GA group were higher than those in the HuA group;the levels of IL-10 in the GA and HuA groups and the levels of IL-4 in the GA group were significantly lower than those of the control group,the level of IL-10 in the GA group was significantly lower than that of the HuA group(all P<0.05);Spearman correlation analysis showed that,the UA level of patients in the HuA group was significantly positively correlated with the ESR,TNF-α,IL-2 level(r=0.449、0.452、0.398),and significantly negatively correlated with IL-10 and IL-1βlevels(r=-0.359,-0.387);UA levels of patients in the control group were significantly negatively correlated with TNF-αlevels,and significantly positively correlated with IL-1βlevels(r=-0.339,0.387)(all P<0.05).Conclusion High UA levels can cause the changes in pro-inflammatory cytokines and anti-inflammatory factors.During the acute attack of GA,the increase of UA levels will neither affect inflammation indicators nor cause changes in inflammatory cytokine levels.
