利用DEA等多方法动态优化科室床位配置的效果评价 被引量:7

Multiple Methods such as DEA Applied to Dynamically Optimize the Department Bed Configuration and Its Effect Evaluation

摘要 目的通过动态优化临床科室床位配置,使医院的医疗资源利用最大化。方法以某三甲综合性医院2018年1月1日-2018年12月31日数据为基础,根据医院实际发展需要及相关政策规定,基于病床工作效率指标计算各临床科室床位的合理区间,结合数据包络分析方法计算各科室床位的调整值并加以实施。将2019年1月1日-2019年12月31日的综合效率最优率、纯技术效率最优率和规模报酬递减率与2018年比较以评价优化效果。结果根据综合性的优化方案对部分科室床位数做出调整,产科、耳鼻喉科等9个科室增加一定数量的床位;妇科和呼吸内科等18个科室减少一定数量的床位。2019年的综合效率最优率为26.67%、纯技术效率最优率为33.33%、规模报酬递减率为26.67%,明显好于2018年相应的评价指标。结论利用病床工作效率指标,结合DEA方法的优化配置床位策略,优化效果显著,可为医院动态调整临床科室床位数提供科学依据。 Objective Dynamically optimizing the allocation of beds in clinical departments to maximize the utilization of hospital medical resources.Methods The adjustment value of each department bed is calculated and implemented based on the 2018 data of a top three comprehensive hospital,according to the actual development needs of the hospital and relevant policy regulations,the reasonable interval of each clinical department bed calculating based on sickbed work efficiency,and the data envelope analysis.The optimization effects are evaluated by comparing the optimal rate of comprehensive efficiency,the effective rate of pure technical efficiency and the rate of diminishing returns to scale in 2019 with 2018.Results According to the comprehensive optimization plan,some departments were adjusted the number of beds.There were 9 departments such as obstetrics and otolaryngology added a certain number of beds;18 departments including gynecology and respiratory medicine reduced a certain number of beds.In 2019,the optimal rate of comprehensive efficiency was 26.67%,the effective rate of pure technical efficiency was 33.33%,and the rate of decreasing returns to scale was 26.67%,which was better than the corresponding evaluation index in 2018.Conclusions The strategies of bed allocation proposed according to sickbed work efficiency index combined with the DEA method significant optimize the allocation of clinical department beds,provide a scientific basis for the hospital to dynamically adjust the number of clinical department beds,and lay the foundation for the hospital’s philosophy of"One bed in the whole hospital".
