糖尿病足感染患者病原菌分布及耐药情况分析 被引量:2

摘要 目的:研究糖尿病足感染患者病原菌分布及耐药情况。方法:2017年3月-2019年3月收治糖尿病足感染患者80例,观察常见病原菌类别、分布及各种病原菌不同的耐药情况。结果:共分离出病原菌87株,其中包含革兰阳性菌38株(43.6%),革兰阴性菌27株(31.0%),真菌17株(19.5%),厌氧菌5株(5.7%)。其中革兰阳性菌对万古霉素、诺氟沙星、环丙沙星及苯唑西林有着较高的敏感性。革兰阴性菌对头孢泊肟、丁胺卡那霉素、亚胺培南的敏感性相对高,对青霉素、头孢曲松、红霉素、氨苄青霉素及加替沙星的敏感性较低,而敏感性最低的是氨苄青霉素。真菌对卡泊芬净的敏感性最高,对氟康唑敏感性最低。厌氧菌对甲硝唑和替硝唑的敏感性最高,对红霉素、甲砜霉素及四环素的敏感性最低。结论:糖尿病足感染的病原菌主要是革兰阳性菌,革兰阳性菌分布较广,应及时进行药敏试验,选取比较敏感的抗生素,对临床治疗效果有着重要意义。 Objective:To study the distribution of pathogenic bacteria and drug resistance in patients with diabetic foot infection.Methods:From March 2017 to March 2019,80 cases of patients with diabetic foot infection were enrolled,the types and distribution of common pathogens and the drug resistance of various pathogens were observed.Results:87 strains of pathogenic bacteria were isolated,included 38 cases of Gram-positive bacteria(43.6%),27 cases of Gram-negative bacteria(31.0%),17 cases of fungi(19.5%)and 5 cases of anaerobic bacteria(5.7%).Gram-positive bacteria showed high sensitivity to vancomycin,norfloxacin,ciprofloxacin and oxacillin.Gram-negative bacteria had relatively high sensitivity to cefopoxime,amikacin and imipenem.The sensitivity to penicillin,ceftriaxone,erythromycin,ampicillin and gatifloxacin was low,while ampicillin was the lowest.Fungi had the highest sensitivity to caspofungin and the lowest sensitivity to fluconazole.Anaerobic bacteria had the highest sensitivity to metronidazole and tinidazole and the lowest sensitivity to erythromycin,tetracycline and thiamphenicol.Conclusion:The main pathogen of diabetic foot infection is Gram-positive bacteria,Gram-positive bacteria is widely distributed,drug sensitivity test should be carried out in time and more sensitive antibiotics should be selected,which has great significance to effect.
