摘要
目的探讨改良序贯疗法治疗手指关节慢性骨髓炎的临床疗效。方法自2015年12月至2019年2月,我们对21例手指关节慢性骨髓炎患者行病灶扩创,术中关节行可吸收线"十"字固定融合,术毕放置自制闭式冲洗管,术后1周采用改良序贯疗法,即交替使用敏感抗生素及医用生物胶体分散剂冲洗病灶,门诊随访观察伤口及指骨愈合情况。结果所有患者术后2周伤口愈合良好,无红肿及炎性渗出,血常规白细胞计数及C反应蛋白等均正常。术后6~8个月门诊随访,患指未再发红肿、疼痛等异常,复查X线片未见指骨原病灶周围处骨质坏死.骨密度降低等异常,关节融合处骨痂生长良好。结论改良序贯疗法治疗手指关节慢性骨髓炎可获得较满意的临床效果。
Objective To explore the clinical efficacy of improved sequential therapy for the treatment of chronic osteomyelitis of digital joint.Methods From December 2015 to February 2019,21 patients with chronic osteomyelitis of the digital joint were treated with debridement.During the operation,the joint was performed cross fixation and fusion with absorbable line.After the operation,the self-made closed flushing tube was placed.One week after the operation,the improved sequential therapy was used.The focus was washed with sensitive antibiotics and medical biocolloid dispersant alternately.The wound and phalange healing were observed in the outpatient follow-up.Results All the patients achieved good healing 2 weeks after the operation,without swelling and inflammatory exudation.The white blood cell count and C-reactive protein were normal.After 6 to 8 months of outpatient follow-up,no redness,swelling,pain and other abnormalities were found in the affected fingers.No abnormal bone necrosis,bone density reduction and other abnormalities were found in the X-ray film,and callus growth was good in the joint fusion area.Conclusion The improved sequential therapy for chronic osteomyelitis of the digital joint can obtain satisfactory clinical efficacy.
