摘要
[目的]统计研究解放军第960医院18年间脊柱感染患者的收治情况,探讨目前存在的问题及未来面临的挑战。[方法]回顾分析2001年1月1日~2018年12月31日,在本科住院的254例诊断为脊柱感染的患者,按照3年一个时间段,共6个时间段,分别统计患者比率、年龄、手术比率、特异性感染比率和病原检测阴性比率,计算其与时间段的相关性。[结果]随时间段推移,脊柱感染患者所占比率显著增加、年龄呈增大趋势,手术治疗比率呈下降趋势,特异感染比率呈下降趋势,病原检测阳性比率呈下降趋势,不同时间段间差异均有统计学意义(P<0.05)。Pearson相关分析表明:时间段赋值与感染患者数呈正相关(r=0.995,P<0.001),与感染患者年龄呈正相关(r=0.889,P=0.018),与手术比率呈负相关(r=-0.945,P=0.034),与特异性感染比率呈负相关(r=-0.811,P=0.050),与病原学检测阴性率呈正相关(r=0.839,P=0.037)。[结论]随时间推移,脊柱疾病住院人数的增多,其中脊柱感染患者也显著增加,并且年龄增大。但是,手术治疗比例减少,病原检测阳性比率下降。
[Objective] To explore the variations of patients who were admitted to our hospital for spinal infection over the past 18 years to analyze the problems and challenges in future according to the data. [Methods] A retrospective study was conducted on the 254 patients who were diagnosed as spinal infection in our department from January 1, 2001 to December 31,2018. The ratio of spinal infection to the total spinal disorders, age, ratio of surgical intervention, ratio of specific infections, and ratio of positive pathogen test were compared based on 6 time-intervals divided with 3 years in each interval. In addition, correlations between the value of time intervals and other variables were searched. [Results] As time went the number of spinal infection significantly increased(P<0.05), the average age of patients with spinal infection significantly increased(P<0.05), whereas the ratio of surgical treatment, the ratio of specific infection,and the ratio of positive pathogen test significantly decreased(P<0.05). As results of Pearson correlation analysis, the value of time-interval was positively correlated to the number of spinal infection(r= 0.995, P=0.000) and the average age of the patients with spinal infection(r= 0.889, P=0.018), whereas negatively correlated to the ratio of surgical treatment(r=-0.945, P=0.034), to the ratio of specific infection(r=-0.811, P=0.050), and the ratio of positive pathogen test(r= 0.839, P=0.037). [Conclusion] With the increase of the spinal disorders over time, the number of patients with spinal infection is increasing considerably with advance of age. However, the ratio of surgical treatment for spinal infection and ratio of positive pathogen test is decreasing in this hospital.
