摘要
目的:探讨胎儿宫内窘迫与妊娠晚期B族溶血性链球菌(GBS)阳性、人胎盘催乳素(hpl)和雌三醇(E3)水平关系及对妊娠结局的影响。方法:回顾性收集2019年1月—2019年12月在本院产科分娩的产妇150例,其中确诊为胎儿宫内窘迫80例为病例组,正常产妇70例为正常组;比较两组GBS阳性率,以胎儿宫内窘迫为因变量,以GBS阳性、hpl、E3、血清瘦素(LP)为自变量进行logistic回归分析。结果:病例组GBS阳性检测率(58.8%)高于正常组(12.9%),血清hpl(6.26±1.89μg/ml)、E3(12.86±3.67 ng/ml)、LP(20.58±4.27μg/L)水平低于正常组(P<0.05);剖宫产率病例组(18.8%)高于正常组(5.7%),产后出血(6.3%)高于正常组(2.9%)(均P<0.05)。多因素回归分析显示GBS阳性、hpl、E3为胎儿宫内窘迫独立危险因素(P>0.05)。结论:妊娠晚期GBS阳性与血清中hpl、E3水平降低是胎儿宫内窘迫发生的危险因素,严重影响分娩结局。围产期定期检测GBS及血清hpl、E3水平在评价胎盘功能中有重要意义。
Objective:To explore the relationship between the GBS positive rate of women during the third trimester of pregnancy and their human galactagogin(h pl)and E3 levels and their fetal distress situation,and to study the influence of the GBS positive on their pregnancy outcomes.Methods:150 puerperae were included and were divided into 80 women with fetal distress in study group and 70 normal puerperae in control group from January 2019 to December 2019.The GBS positive rate of puerperae was compared between the two groups.Logistic regression analysis was performed with intrauterine distress as the dependent variable,and with GBS positive,hpl level,E3 level,and LP level as independent variables.Results:The positive detection rate of GBS of puerperae in the study group(58.8%)was significant higher than that(12.9%)of puerperae in the control group,and the serum levels of HPL(6.26±1.89 g/ml),E3(12.86±3.67 ng/ml),and LP(20.58±4.27 g/L)were significant lower than those of puerperae in the control group(P<0.05).The cesarean section rate of puerperae in the study group(18.8%)was significant higher than that(5.7%)of puerperae in the control group,and the rate of postpartum hemorrhage of puerperae in the study group(6.3%)was also significant higher than that(2.9%)of puerperae in the control group(all P<0.05).Multivariate regression analysis showed that positive GBS,and HPL level and E3 level were independent risk factors of fetal distress(P>0.05).Conclusion:GBS positive of women during the third trimester of pregnancy and the decreased serum levels of hpl and E3 are risk factors of fetal distress in uterus,which will severely affect the final delivery outcomes.The detection of GBS positive and serum levels of hpl and E3 of women during perinatal period has significance dor evaluating their placental function.
