摘要
目的:通过对本院门急诊尿路感染(urinary tract infection,UTI)处方进行统计分析和评价,了解门急诊UTI抗菌药物使用情况,为临床合理有效地使用抗菌药物提供参考。方法:抽取2019年5月~2020年5月本院UTI门急诊处方810张,统计UTI患者性别、年龄分布、各种抗菌药物用药频度与合理用药情况,分析门急诊UTI抗菌药物使用情况。结果:810张处方中,男性273例(38.72%),女性432例(61.28%),UTI患者中女性多于男性;20~30岁的人数最多(占26.38%);用药频度(DDDs)前5位的是盐酸左氧氟沙星片、头孢地尼胶囊、盐酸克林霉素棕榈酸酯分散片、硫酸依替米星注射液、头孢克肟干混悬剂;不合理处方109张(13.46%),涉及遴选的药品不适宜、联合用药不适宜、重复用药和超说明书用药等。结论:本院门急诊UTI抗菌药物使用存在一些问题,需加强本院门急诊处方审核,促进本院UTI用药更加规范化。
Objective:To understand the use of antibiotics for urinary tract infection(UTI)and provide a reference for the rational and effective use of antibiotics in clinic,through statistical analysis and evaluation of the prescription for UTI in outpatient emergency in a hospital.Methods:810 outpatient emergency prescriptions for UTI were extracted from the hospital from May.2019 to May.2020.The distribution of gender and age of patients in the prescription,the medication frequency of various antibiotics in the prescription and the rational medication were analyzed.Results:There were 273 male patients(38.72%)and 432 female patients(61.28%)in the 810 prescriptions for UTI and the largest age group were between 20 and 30 years old,accounting for 26.38%.Female patients are significantly more than male patients.The top five drugs in the frequency of medication(DDDs)are levofloxacin hydrochloride tablets,cefdinir capsules,clindamycin palmitate hydrochloride dispersible tablets,etimicin sulfate injection and cefixime for oral suspension.There were 109(13.46%)irrational prescriptions for UTI,including inappropriate selection of antibiotics,inappropriate medicine combination,repeated drug use,off-label use of antibiotics.Conclusion:There are some problems in the use of antibiotics for UTI of outpatient emergency in this hospital.It is necessary to strengthen the review of outpatient emergency prescriptions to promote the rationalization of antibiotics use for UTI in this hospital.
