摘要
目的探讨吉兰-巴雷综合征(GBS)患者的神经电生理特点及其临床诊断意义。方法对65例GBS患者进行运动神经传导速度(MCV)、感觉神经传导速度(SCV)、F波、H反射及肌电图(EMG)检测,共检测228条运动神经、195条感觉神经及182块肌肉,并分析结果。结果MCV减慢67.5%,SCV减慢38.5%,上、下肢运动神经远端潜伏期(DML)延长49.1%,F波异常86.1%,H反射异常75.4%,EMG提示神经源性损害占43.1%。结论GBS为广泛的周围神经损害,存在以脱髓鞘为主伴有轴突变性的神经电生理改变,神经电生理检测对GBS的诊断、治疗和预后判断可提供重要参考。
Objective To study the electrophysiological characteristics of Guillain-Barre syndrome(GBS)patients and the significance in clinical diagnosis.Methods The motor Conduction Velocity(MCV),sensory nerve conduction velocity(SCV),F wave,H-reflex,electromyography(EMG)in 65 patients with GBS were detected.There were 228 motor nerves,195 sensory nerves,and 182 muscles which were detected and analyzed.Results MCV slowed by 67.5%,SCV slowed by 38.5%,upper and lower extremity of distal motorial latency(DML)was prolonged by 49.1%,F wave was above normal by 86.1%,and H-reflex was above normal by 75.4%.At the same time,EMG showed that neurogenic damage accounted for 43.1%.Conclusion GBS does a wide range of damage to peripheral nerve,and there is demyelination together with axonal degeneration of neural electrophysiological changes.Neurophysiological testing can provide important value for the diagnosis,treatment and prognosis of GBS.
