摘要
目的:观察不同电针时间的低频电针治疗中风后吞咽障碍的临床疗效,为临床电针治疗中风后吞咽障碍的时间参数提供客观参考依据。方法:选取中风后吞咽障碍患者54例,采用随机数字表法按照就诊先后顺序将其分为观察组1(电针15分钟)和观察组2(电针30分钟)各27例。在常规治疗基础上,两组患者电针选取廉泉、风府穴,频率为2 HZ的低频连续波,强度以患者能耐受的最大强度,观察组1电针时间为15分钟,观察组2电针时间为30分钟。每日治疗1次,每周5次,共2周。治疗前及治疗2周后采用标准吞咽功能评价法(SSA)、洼田饮水试验、微型营养评价法(MNA)评价患者的吞咽功能及营养状况。结果:两组患者治疗前一般情况及SSA、洼田饮水试验、MNA评分、MNA分级均无统计学差异(P>0.05)。两组患者治疗后SSA评分、洼田饮水试验、MNA评分、MNA分级均较治疗前改善(P<0.05)。治疗后,观察组1的SSA评分、洼田饮水试验、MNA评分、MNA分级均明显优于观察2组,且差异有统计学意义(P<0.05)。结论:电针廉泉、风府穴治疗中风后吞咽障碍,在2 HZ低频连续波、患者能耐受的最大强度的电针参数下,电针时间15分钟较30分钟疗效更佳。
Objective:To observe the clinical effects of dysphagia after stroke treated by low frequency electroacupuncture with different duration,and provide objective basis for the duration of electroacupuncture in treating dysphagia after stroke.Methods:fifty-four patients who suffered from dysphagia after stroke have been selected according to eligible criteria.They are randomly divided into two working groups:Observation group 1 the group of 15 minutes electroacupuncture,the other group is observation group 2,with 30 minutes electroacupuncture,27 patients each.Two groups are conducted on the basic of routine therapies.Both of the two groups are under 2 HZ low frequency continuous wave electroacupuncture treat on LianQuan and Fengfu points,and intensity is the maximum strength a patient can tolerate.The treatment schedule is once a day,5 visits per week,totally 2 weeks.Standardized Swallowing Assessment(SSA)、Watian swallowing test and Mini Nutrition Assessment(MNA)will be used to assess the therapeutic effect of swallowing function and nutritional status before and after the treatment.Results:Comparision of the SSA、Watian swallowing test and MNA before the treatment,there are no significant differences between two groups(P>0.05).Compared to pretherapy,both of the two groups,the scores of SSA、Watian swallowing tes、scores of MNA and MNA grade are significantly improved after the treatment(P<0.05).After the treatment,the score of SSA、Watian swallowing test、scores of MNA and MNA grade of the observation group 1 are superior to the observation group 2 after the treatment(P<0.05).Conclusions:Electroacupuncture on LianQuan and Fengfu points with parameters of 2 HZ low frequency continuous wave,and the maximum strength a patient can tolerate,15 minutes electroacupuncture works more effective than 30 minutes in patients who suffered from dysphagia after stroke.
