摘要
目的:探究CT能谱成像联合MRI弥散加权成像在肝癌TACE术后随访中的应用。方法:收集2017年1月~2020年5月我院诊治行TACE术的肝癌患者共78例,术后6周采取CT能谱成像与MRI弥散加权成像检测,应用DSA作为金标准,CT能谱成像联合MRI弥散加权成像在肝癌TACE术后随访复发及转移诊断价值。结果:DSA提示78例随访复查经TACE术治疗肝癌患者中MRI或GSI检查提示115个病灶,阴性病灶31个,阳性病灶84个。GSI显示存在75个阳性病灶,其敏感性、特异性、准确性、阳性预测值与阴性预测值分别为80.95%、77.42%、80.00%、91.02%、78.00%。DWI检测提示存在75个阳性病灶,其敏感性、特异性、准确性、阳性预测值与阴性预测值分别为89.29%、80.65%、86.70%、93.33%、73.11%。联合检测敏感性、特异性、准确性、阳性预测值与阳性预测值分别为100%、90.32%、98.25%、96.57%、100%。与GSI与DWI单独检测相比,联合检测的敏感性、特异性及准确性均较高(P<0.05)。结论:CT能谱成像联合MRI弥散加权成像在TACE术后随访中能提示病灶残存与复发情况,适宜应用推广。
Objective:To explore the application of CT energy spectrum imaging combined with MRI diffusion weighted imaging in the follow-up of hepatocellular carcinoma after TACE.Methods:a total of 78 patients with liver cancer who underwent TACE in our hospital from January 2017 to May 2020 were collected.Six weeks after operation,CT energy spectrum imaging and MRI diffusion-weighted imaging were detected.DSA was used as the gold standard.The diagnostic value of CT energy spectrum imaging combined with MRI diffusion weighted imaging in the follow-up of recurrence and metastasis of liver cancer after TACE was evaluated.Results:DSA indicated that in 78 patients with liver cancer treated by TACE,MRI or GSI examination revealed 115 lesions,31 negative lesions and 84 positive lesions.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value were 80.95%,77.42%,80.00%,91.02%and 60.00%,respectively.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of DWI were 89.29%,80.65%,86.70%,93.33%and 73.11%,respectively.The sensitivity,specificity,accuracy,positive predictive value and positive predictive value were 100%,90.32%,98.25%,96.57%and 100%,respectively.Compared with GSI and DWI alone,the sensitivity,specificity and accuracy of combined detection were higher(P<0.05).Conclusion:CT energy spectrum imaging combined with MRI diffusion weighted imaging in the follow-up after TACE can show the residual and recurrence of lesions,which is suitable for application and promotion.
